摘　　要：目的:研究集中带量采购对冠脉支架置入术患者住院费用的影响,为深化高值医用耗材改革提供依据。方法:收集某三甲医院集中带量采购前后冠脉支架置入术患者病案首页数据,回顾性分析住院费用。结果:集中带量采购后样本医院冠脉支架置入术出院患者人数比集中带量采购前同期增长19.28%,均次住院费用同比下降43.53%,手术用一次性医疗耗材费用同比下降64.42%;单因素和多因素分析分别显示,影响住院费用的首要因素是入院病情严重程度和住院天数。结论:冠脉支架集中带量采购效果明显,但耗材费在住院费用中的占比依然最高,需三医联动,持续推进高值医用耗材集中带量采购工作。Objective To study the changes of hospitalization expenses of the patients undergoing coronary stent implantation before and after the implementation of centralized volume-based procurement policy,and to provide evidence for deepening the payment reform of high-value consumables.Methods The first page data of medical records before and after the implementation of centralized volume-based procurement policy in a first class hospital were collected,and the hospitalization expenses were analyzed retrospectively.Results After the implementation of centralized volume-based procurement,the number of patients undergoing coronary stent implantation increased by 19.28%,the average hospitalization expenses decreased by 43.51%,and the cost of disposable medical consumables for surgery decreased by 64.42%.Univariate analysis showed that the more critically the admission condition,the higher the cost of hospitalization.Multivariate analysis showed that the primary factor affecting hospitalization cost was the length of stay.Conclusion The effect of centralized volume-based procurement in controlling hospitalization expenses is significant,but the proportion of consumables costs in hospital expenses is still the highest.It is necessary to continue to promote the centralized volume-based procurement of high-value medical consumables through joint reform of medical service,medication and medical assurance.
