基于核磁共振评价宫廷理筋振髌法治疗膝关节骨关节炎的临床疗效

摘　　要：目的 利用核磁共振观察宫廷理筋振髌法治疗膝关节骨关节炎(knee osteoarthritis,KOA)的临床疗效。方法 回顾东直门医院收治的膝关节骨关节炎患者44例,其中按摩组22例,用宫廷理筋振髌法治疗,中药组22例,用东直门医院骨科洗药治疗,2组患者治疗前均进行核磁共振检查,3~6个月治疗期间复查核磁共振,观察前交叉韧带、后交叉韧带、髌韧带、内侧副韧带、外侧副韧带、髌下脂肪垫、内侧支持带、外侧支持带、半月板损伤、关节积液、髌骨软化等部位治疗前后变化情况。结果 按摩和中药治疗膝关节骨关节炎均有效,按摩组患者临床疗效优于中药组(P<0.05)。结论 宫廷理筋振髌法治疗膝关节骨关节炎患者临床疗效理想,患者易于接受,同时核磁共振检查(magnetic resonance imaging,MRI)在膝关节骨关节炎的诊断分期、治疗、疗效评价等方面有明显的优势。Objective To observe the clinical effect of the method of Regulating Tendons and Vibrating Patella in Palace on knee osteoarthritis by using unclear magnetic resonance imaging(MRI).Methods A total of 44 patients with knee osteoarthritis were treated in Dongzhimen Hospital,including 22 patients in massage group treated with patella vibration therapy and 22 patients in traditional Chinese medicine group treated with orthopaedic lotion,the patients in both groups were examined by MRI before and during 3~6 months of treatment,the changes of anterior cruciate ligament,posterior cruciate ligament,patellar ligament,medial collateral ligament,lateral collateral ligament,infrapatellar fat pad,medial retinaculum,lateral retinaculum,meniscus injury,joint effusion and patella malacia were observed before and after treatment.Results Both the massage group and the traditional Chinese medicine group were effective in the treatment of knee osteoarthritis.The clinical effect of massage group was better than that of traditional Chinese group(P<0.05).Conclusion The method of Regulating Tendons and Vibrating Patella in Palace has an ideal clinical effect on the patients with knee osteoarthritis,which is easily accepted by the patients and doctors.At the same time,MRI has obvious advantages in the diagnosis,staging,treatment and curative effect evaluation of knee osteoarthritis.
