首发脑卒中患者溶栓康复期复发恐惧水平现状及其影响因素调查

摘　　要：目的探究首发脑卒中患者经溶栓治疗后康复期的复发恐惧水平及其影响因素。方法选择医院2020年1月—2021年12月收治的接受溶栓治疗的首发脑卒中患者共160例为研究对象,采用一般资料问卷调查患者基础信息,借助恐惧疾病进展简化量表进行调查,通过单因素分析、多因素Logistic回归分析筛选首发脑卒中患者经溶栓治疗后康复期复发恐惧水平的影响因素。结果量表评价得出,首发脑卒中患者经溶栓治疗后康复期的复发恐惧评分为32.50±8.03分,根据恐惧疾病进展简化量表评分对患者进行分组,其中复发恐惧组54例,无复发恐惧组106例。单因素分析得到,首发脑卒中患者经溶栓治疗后康复期复发恐惧水平的影响因素有年龄、文化水平、家庭人均月收入、改良的Rankin量表(MRS)评分、身边复发病例、延续性护理宣教、家庭关怀度、领悟社会支持、疾病不确定感(P<0.05);多因素Logistic回归分析显示,首发脑卒中患者经溶栓治疗后康复期复发恐惧水平的影响因素有初中及以下文化水平、家庭人均月收入<3000元、MRS≥3分、身边有复发病例、未接受延续性护理宣教、家庭关怀度<7分、领悟社会支持<37分、疾病不确定感≥75分(P<0.05)。结论首发脑卒中患者经溶栓治疗后康复期伴有不同程度的复发恐惧情绪,初中及以下文化水平、家庭人均月收入<3000元、MRS≥3分、身边有复发病例、未接受延续性护理宣教、家庭关怀度指数量表评分<7分、领悟社会支持量表评分<37分、疾病不确定感量表评分≥75分的首发脑卒中患者溶栓康复期发生复发恐惧的可能性较大,临床需强化护理干预。Objective To explore the level of fear of recurrence and its influencing factors in first-episode stroke patients after thrombolysis treatment during the recovery period.Methods A total of 160 first-episode stroke patients admitted to the hospital from January 2020 to December 2021 who received thrombolysis treatment were selected as the research subjects.A general information questionnaire was used to investigate the basic information of patients,and a simplified scale for fear of disease progression was used for investigation.Univariate analysis and multivariate Logistic regression analysis were used to screen the influencing factors of relapse fear level in first-episode stroke patients after thrombolysis treatment during the recovery period.Results According to the evaluation of the scale,the fear of recurrence score of first-episode stroke patients during the rehabilitation period after thrombolysis treatment was 32.50±8.03 points.The patients were grouped according to the simplified scale score of fear of disease progression,resulting in a recurrence fear group of 54 cases and a non recurrence fear group of 106 cases.Univariate analysis showed that the influencing factors on the level of fear of recurrence in first-episode stroke patients after thrombolysis treatment during the recovery period include the age,education level,family average monthly income,modified Rankin scale(MRS)score,recurrent cases around them,continuing care education,degree of family care,perceived social support,and sense of disease uncertainty(P<0.05).Multivariate Logistic regression analysis showed that the influencing factors on the level of fear of recurrence in first-episode stroke patients after thrombolysis treatment during the recovery period include the junior high school education level or below,family average monthly income<3000 yuan,MRS score≥3points,have recurrent cases around them,have not received continuing care education,degree of family care<7 points,perceived social support<37 points,and disease uncertainty≥75
