许斌六经辨治亚急性甲状腺炎经验

摘　　要：许斌认为亚急性甲状腺炎为外感内伤共同致病,外感多为风温阳邪始犯太阴之表,继可内传阳明、少阳,七情内伤太、少二阴之里,而生气滞、血瘀、痰湿之有形之邪循经上结于颈前,根据疾病的临床表现,提出初犯太阴、顺传阳明、止于少阳、太少二阴具虚的疾病传变顺序。本病病机关键为风温侵袭,痰气瘀热互结。治疗原则总属急则治标、缓则治本,以散阳邪、清内热、行气化痰活血、内补二阴之虚为治疗之法,并在脉证结合基础上分经论治。Professor Xu Bin believes that subacute thyroiditis is jointly caused by exogenous diseases and internal damages.The former mostly refers to wind-warm diseases,which are caused by yang pathogen first invading the surface of Taiyin,and later internally transmitting to Yangming and Shaoyang,while the internal damages induced by the excess of seven emotions can cause internal injuries to Taiyin and Shaoyin,which may generate the tangible pathogens such as qi stagnation,blood stasis and phlegm-damp that run upward along meridians and accumulate in the upper part of the neck.Based on the clinical manifestations of subacute thyroiditis,Xu Bin puts forward its transmission sequence,that is,both exogenous diseases and internal damages first offend Taiyin,then transmit to Yangming,finally end up with deficient Shaoyang,Taiyin and Shaoyin.The keys to its pathogenesis lie in the invasion of wind-warm diseases and the intermingled phlegm-damp,blood stasis,qi stagnation and heat.Its governing treatment principle features a symptomatic treatment in acute conditions and a radical treatment in chronic cases.Its treatment methods involves dispersing yang pathogen,clearing internal heat,activating qi-flowing,eliminating dampness and resolving phlegm,promoting blood circulation,and treating the deficiency syndromes of Taiyin and Shaoyin with tonifying methods.And the disease should be treated by differentiating channel syndromes on the basis of combining various pulse manifestations and syndromes.
