摘　　要：目的探讨和分析采用中药热奄包治疗小儿急性肠系膜淋巴结炎中寒型的治疗疗效。方法选择84例中寒型急性肠系膜淋巴结炎的病例,随机分为对照组(常规西医抗感染或抗病毒治疗),观察组(在对照组的基础上联合采用中药热奄包治疗),每组42例。治疗2周后,比较2组临床总疗效,中医证候积分,临床症状缓解时间、淋巴结大小变化及安全性评价。跟踪随访1个月,对比分析2组疾病复发情况。结果通过对比分析,观察组临床总有效率(95.24%,40/42)高于对照组(83.33%,35/42)(P<0.05)。治疗后2组的主症和次症得分较治疗前均降低(P<0.05),且观察组低于对照组(P<0.05);患儿腹痛消失时间、体温、食欲、大便恢复正常时间比较,观察组均短于对照组(P<0.05);2组肠系膜淋巴结长轴直径、短轴直径、纵横比较治疗前均缩小(P<0.05),且观察组小于对照组(P<0.05)。跟踪随访1个月,观察组复发率低于对照组(P<0.05)。治疗期间2组均未出现不良反应。结论中药热奄包治疗中寒型小儿急性肠系膜淋巴结炎安全有效,能明显改善患儿临床症状,缩短治疗病程,缩小肿大的淋巴结,降低疾病复发率。Objective To investigate and analyze the therapeutic effect of traditional Chinese medicine hot package in the treatment of acute mesenteric lymphadenitis(with a cold pattern)in children.Methods A total of 84 children of acute mesenteric lymphadenitis with the syndrome of cold were randomly divided into a control group(conventional western medicine anti-infection or antivirus treatment)and an observation group(combined with the traditional Chinese medicine hot package treatment on the basis of the control group),with 42 cases in each group.After 2 week of treatment,the total clinical efficacy,TCM syndrome scores,clinical symptom relief time,lymph node size changes and safety evaluation were compared between the two groups.The disease recurrence was compared and analyzed between the two groups after a one-month follow-up.Results Through a comparative analysis,the total clinical effective rate of the observation group(95.24%,40/42)was higher than that of the control group(83.33%,35/42)(P<0.05).After treatment,the scores of primary and secondary symptoms in the two groups were decreased than those before treatment(P<0.05),and the observation group was lower than the control group(P<0.05).The disappearance time of abdominal pain,the time of body temperature,appetite and stool returning to normal in the observation group were shorter than those in the control group(P<0.05).The diameters of long axis,short axis and longitudinal axis of mesenteric lymph nodes in the two groups were decreased than those before treatment(P<0.05),and the observation group was smaller than the control group(P<0.05).The recurrence rate of the observation group was 5.00%,which was lower than 23.08%of the control group(P<0.05).There were no adverse reactions in the two groups during treatment.Conclusion Traditional Chinese medicine hot package is safe and eflective in the treatment of acute mesenteric lymphadenitis with the syndrome of cold in children,which can significantly improve their clinical symptoms,shorten the treatment course,shrink
