超声在主动脉内球囊反搏导管定位中的应用价值

摘　　要：目的 针对主动脉内球囊反搏(IABP)置管术中床旁重症超声定位导管位置效果展开分析,探讨超声定位技术的准确性,为临床IABP的开展提供参考。方法 选取2019年1月至2022年12月我院收治的89例行IABP置入的急性心肌梗死并心源性休克患者作为研究对象,采用自身对照研究,将研究对象超声定位确定的导管位置与置管结束后床旁X线确定的位置进行比较。结果借助重症超声技术定位导管位置效果较好,与胸部X线定位IABP导管位置一致性较高,差异无统计学意义(P> 0.05);超声定位IABP导管的准确度为94.38%,灵敏度为86.62%,一致性检验Kappa值为0.782。结论 超声定位与传统X线定位IABP导管相比具有较高的准确性、实时性、经济性,具有临床应用价值。Objective To analyze the position effect of bedside severe ultrasound positioning catheter during intracortical balloon inversion stroke(IABP)catheterization,explore the accuracy of ultrasound positioning technology,and provide a reference for the development of clinical IABP.Methods All 89 patients with acute myocardial infarction and cardiogenic shock admitted to our hospital from January 2019 to December 2022 were selected as the study subjects,using a self-controlled study.The catheter position determined by ultrasound positioning of the study subjects was compared with the position determined by the bedside X-ray after the end of tube placement.Results The location of the catheter with the help of intensive ultrasound technology was good,and the consistency with the positioning of IABP catheter by chest X-ray was high,and the difference was not statistically significant(P>0.05),the accuracy of ultrasound positioning of IABP catheter was 94.38%,the sensitivity was 86.62%,and the Kappa value of consistency test was 0.782.Conclusion Compared with the traditional X-ray positioning IABP catheter,severe ultrasound positioning has high accuracy,real-time and economy,and has clinical application value.
