摘 要:目的探讨多部门综合干预,对提高临床病原微生物标本送检质量的效果。方法选取某三甲医院2020年—2021年治疗性使用抗菌药物的住院患者,2020年12月开始实施综合干预措施,比较干预前组(2020年1月—12月)和干预后组(2021年1月—12月)住院患者抗菌药物治疗前微生物标本送检率、不同类型微生物标本构成及血培养标本送检情况。结果共调查治疗性使用抗菌药物20530例,干预前9096例,干预后11434例。干预后抗菌药物、限制级抗菌药物、特殊级抗菌药物治疗前微生物标本送检率分别为76.32%、82.59%、94.76%,高于干预前的65.6%、68.36%、89.5%,差异有统计学意义(P<0.01)。无菌部位标本由干预前的17.72%提高至干预后的20.95%。2套以上血培养标本由干预前的58.73%提高至干预后的62.75%,差异有统计学意义(P<0.01)。结论应用综合干预措施,可提高微生物标本送检率和送检质量。Objective This paper aims to explore the effect of multi-department comprehensive intervention on improving the quality of clinical microbial specimens.Methods Patients who used therapeutic antimicrobial agents from 2020 to 2021 in a tertiary A hospital were selected,and comprehensive intervention measures were implemented from December 2020.Submission rates of microbial specimens,different types of specimens and blood culture specimens of inpatients before(January to December,2020)and after intervention(January to December,2021)were compared.Results A total of 20530 cases were investigated,including 9096 cases before intervention and 11434 cases after intervention.After the intervention,the detection rates of microbial specimens before antimicrobial therapy,restrictive antimicrobial therapy,and special antimicrobial therapy were 76.32%,82.59%and 94.76%,which were higher than that before intervention(65.6%,68.36%and 89.5%),with the differences statistically significant(P<0.01).The proportion of sterile specimens increased from 17.72%before interference to 20.95%after interference.The proportion of two or more sets of blood culture specimens increased from 58.73%before intervention to 62.75%after intervention,with the differences statistically significant(P<0.01).Conclusion Comprehensive intervention measures can improve the detection rate and quality of microbial specimens.
