摘  要:目的:研究补肾降浊饮对非酒精性脂肪性肝炎(Non-alcoholic Steatohepatitis,NASH)大鼠肠道菌群及磷脂酰肌醇-3-激酶(Phosphatidylinositol 3-kinase,PI3K)/丝氨酸/苏氨酸蛋白激酶(RAC-alpha Serine/Threonine-protein Kinase,Akt)/核因子-κB(Nuclear Factor-κB,NF-κB)信号通路的影响,探讨补肾降浊饮改善NASH的机制。方法:将40只SD大鼠随机分为空白组、模型组、补肾降浊饮组及西药组。空白组予普通饲料喂养,其余各组给予高脂饲料喂养,并分别灌胃生理盐水或相应药物干预。12周后检测各组大鼠血清胆固醇(Total Cholesterol,TC)、三酰甘油(Triglyceride,TG)、谷丙转氨酶(Alanine transaminase,ALT)、谷草转氨酶(Aspartate aminotransferase,AST)及肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α)、白细胞介素-6(Interleukin-6,IL-6)水平;油红O染色检测肝组织脂肪变性程度;RT-PCR法检测粪便中大肠杆菌、乳酸杆菌含量;Western-blot法检测肝组织中PI3K、Akt、NF-κB表达水平。结果:各药物组均可不同程度改善大鼠血清中TC、TG、ALT、AST、TNF-α、IL-6含量及肝组织PI3K、Akt、NF-κB表达水平(P<0.05,P<0.01);补肾降浊饮组虽在改善ALT、AST及TNF-α、IL-6含量及Akt蛋白表达方面与西药组无明显差异,但在改善TC、TG、油红O染色、肠道菌群及PI3K、NF-κB方面优于西药组。结论:补肾降浊饮改善NASH机制可能与调节肠道菌群进而影响PI3K/Akt/NF-κB信号通路传导以减少TNF-α、IL-6等炎性因子的产生有关。Objective:To study the effect of Bushen Jiangzhuo Yin(补肾降浊饮)on the intestinal flora and PI3K/Akt/NF-κB signaling pathway of NASH rats,and to explore the mechanism of Bushen Jiangzhuo Yin on NASH.Methods:Forty SD rats were randomly divided into the blank group,the model group,the Bushen Jiangzhuo Yin group and the Western medicine group.Gastric saline or corresponding dose of drug intervention was given.After 12 weeks,TC,TG,ALT,AST,TNF-α,and IL-6 levels were detected;Oil red O staining was used to detect the degree of steatosis in liver tissue;RT-PCR method was used to detect the content of escherichia coli and lactobacillus in stool;Western-blot method was used to detect PI3K,Akt,NF-κB expression levels in liver tissue.Results:Each drug group could ameliorate the levels of TC,TG,ALT,AST,TNF-α,IL-6 in serum and the expression levels of PI3K,Akt,NF-κB in liver tissue of rats to varying degrees(P<0.05,P<0.01);Although the Bushen Jiangzhuo Yin group had no significant difference with the western medicine group in ameliorating the contents of ALT,AST,TNF-α,IL-6 and Akt protein expression,but in ameliorating the TC,TG,Oil Red O staining,intestinal flora and PI3K,NF-κB was better than the western medicine group.Conclusion:The mechanism of Bushen Jiangzhuo Yin in improving NASH may be related to the regulation of intestinal flora,thereby affecting the transduction of PI3K/Akt/NF-κB signaling pathway to reduce the production of inflammatory factors such as TNF-αand IL-6.
