余绍清教授从心肝论治双心疾病的临床经验

摘　　要：随着人民生活水平不断提高、工作节奏加快,人们的精神压力不断增加,越来越多的人出现心悸、烦躁不安、不寐等一系列心理及心脏疾患,给患者的健康和日常工作和生活等带来较大影响。1995年,“双心医学”的概念在国内被首次提出,它强调在临床诊疗心脏疾病时应同时关注心血管疾病和心理问题即双心疾病。双心疾病是指心血管疾病伴有焦虑抑郁等心理疾病。余绍清教授认为焦虑、抑郁等情志因素与肝脏密切相关。文章分别从心肝的五行属性、藏象属性及情志联系等方面出发,阐述阴阳失衡、经络失和、君相失调等心肝因素是导致双心疾病的发病基础,在治疗方面,辨证论治,心肝同治,以肝为重,辅以心理疏导,从而达到治愈疾病的目的。With the improvement of people’s living standard and the acceleration of the pace of work,people's mental pressure is increasing year by year,more and more people have a series of psychological and heart diseases such as palpitation,irritability,and insomnia,which have a great impact on the health of patients and their daily work and life.In 1995,the concept of psychocardiology medicine was first put forward in China,which emphasizes that cardiovascular disease and psychological problems should be paid attention to in the clinical diagnosis and treatment of heart diseases,namely,cardiovascular and cerebrovascular disease with psychological abnormality.Cardiovascular and cerebrovascular disease with psychological abnormality refers to cardiovascular disease accompanied by mental illness such as anxiety and depression.Professor YU Shaoqing believes that emotional factors such as anxiety and depression are closely related to the liver.In this article,from the perspectives of the five-element attributes,visceral manifestation attributes,and emotional connection of the heart and liver,it is expounded that the heart and liver factors such as the imbalance of Yin(阴)and Yang(阳),the disharmony of the meridians,and the incongruity of the monarch and phase are the basic pathogenesis of cardiovascular and cerebrovascular disease with psychological abnormality.In terms of its treatment,based on syndrome differentiation,the heart and liver are treated simultaneously,and the liver is the most important,supplemented by psychological counseling,so as to achieve the purpose of curing the disease.
