屈螺酮炔雌醇片与左炔诺孕酮宫内节育系统联合治疗子宫内膜增生不伴非典型增生的效果观察

摘　　要：目的探究屈螺酮炔雌醇片与左炔诺孕酮宫内节育系统联合治疗子宫内膜增生不伴非典型增生的效果。方法100例子宫内膜增生不伴非典型增生患者作为研究对象,按照电脑随机法分为观察组和对照组,每组50例。对照组采用左炔诺孕酮宫内节育系统治疗,观察组在对照组基础上加用屈螺酮炔雌醇片治疗。对比两组治疗前后子宫内膜厚度、月经周期以及月经期,不良反应发生情况,治疗前后卵巢储备功能指标[窦卵泡计数(AFC)、卵泡刺激素(FSH)、促黄体素(LH)、雌二醇(E_(2))]。结果治疗前,两组患者的子宫内膜厚度、月经期、月经周期对比差异无统计学意义(P>0.05);治疗后,两组患者的子宫内膜厚度小于本组治疗前,月经期、月经周期均短于本组治疗前,观察组子宫内膜厚度(5.25±1.02)mm小于对照组的(7.52±1.74)mm,月经期(4.51±1.01)d、月经周期(28.52±1.41)d均短于对照组的(7.52±1.52)、(30.85±1.47)d,差异具有统计学意义(P<0.05)。观察组患者的不良反应发生率2.00%显著低于对照组的20.00,差异具有统计学意义(P<0.05)。两组治疗前后组间AFC、FSH、LH、E_(2)水平对比差异均无统计学意义(P>0.05)。结论子宫内膜增生不伴非典型增生患者采用屈螺酮炔雌醇片与左炔诺孕酮宫内节育系统联合治疗的效果显著,且安全性高,值得研究和推广。Objective To investigate the effect of drospirenone and ethinylestradiol tablets combined with levonorgestrel intrauterine system in the treatment of endometrial hyperplasia without atypical hyperplasia.Methods A total of 100 cases of endometrial hyperplasia without atypical hyperplasia were divided into observation group and control group according to computer randomized method,with 50 cases in each group.The control group was treated with levonorgestrel intrauterine system,and the observation group was treated with drospirone and ethinylestradiol tablets on the basis of the control group.Both groups were compared in terms of endometrial thickness,menstrual cycle and menstrual period,occurrence of adverse reactions,and ovarian reserve indicators[antral follicle count(AFC),follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E_(2))]before and after treatment.Results Before treatment,there were no statistically significant differences in endometrial thickness,menstrual period and menstrual cycle between the two groups(P>0.05).After treatment,the endometrial thickness in the two groups was less than that before treatment in this group,and the menstrual period and menstrual cycle were shorter than those before treatment in this group;the endometrial thickness of(5.25±1.02)mm in the observation group was less than that of(7.52±1.74)mm in the control group;the menstrual period of(4.51±1.01)d and menstrual cycle of(28.52±1.41)d in the observation group were shorter than those of(7.52±1.52)and(30.85±1.47)d in the control group;the differences were statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 2.00%,which was significantly lower than that of 20.00% in the control group,and the difference was statistically significant(P<0.05).There were no statistically significant differences in the levels of AFC,FSH,LH and E_(2) between the two groups before and after treatment(P>0.05).Conclusion The combined treatment of drospirenone and ethinylestradiol tablets a
