摘 要:目的研究甲钴胺联合α-硫辛酸治疗糖尿病周围神经病变对神经电生理的影响与相关机制。方法104例糖尿病周围神经病变患者,根据治疗方式不同分为实验组与对照组,每组52例。对照组接受甲钴胺治疗,实验组在对照组基础上接受α-硫辛酸治疗。比较两组患者治疗效果以及治疗前后神经电生理指标(运动神经传导速度、感觉神经传导速度)、氧化应激指标[超氧化物歧化酶(SOD)、丙二醛(MDA)]。结果实验组治疗总有效率96.15%高于对照组的84.62%,差异具有统计学意义(P<0.05)。治疗后,实验组患者正中神经运动神经传导速度(55.12±3.01)m/s、腓总神经运动神经传导速度(49.86±2.89)m/s、正中神经感觉神经传导速度(48.72±2.25)m/s、腓总神经感觉神经传导速度(45.86±2.56)m/s均高于对照组的(47.12±2.96)、(42.67±1.69)、(42.09±1.79)、(38.12±2.61)m/s,差异均具有统计学意义(P<0.05)。实验组患者SOD(134.42±6.80)U/ml高于对照组的(125.09±6.26)U/ml,MDA(4.09±0.73)nmol/ml低于对照组的(5.88±0.60)nmol/ml,差异均具有统计学意义(P<0.05)。结论运用甲钴胺联合α-硫辛酸治疗糖尿病周围神经病变患者效果显著,可以显著改善患者的神经电生理指标,具有较高的疗效,可推广。Objective To study the effect of mecobalamin combined withα-lipoic acid on neuroelectrophysiology of diabetic peripheral neuropathy and its related mechanism.Methods A total of 104 patients with diabetic peripheral neuropathy were divided into experimental group and control group according to different treatment methods,with 52 cases in each group.The control group was treated with mecobalamin,and the experimental group was treated withα-lipoic acid on the basis of the control group.Both groups were compared in terms of therapeutic effect,neuroelectrophysiological indexes(motor nerve conduction velocity,sensory nerve conduction velocity)and oxidative stress indexes[superoxide dismutase(SOD),malondialdehyde(MDA)]before and after treatment.Results The total effective rate of the experimental group was 96.15%,which was higher than that of 84.62% of the control group,and the difference was statistically significant(P<0.05).After treatment,the experimental group had median motor nerve conduction velocity of(55.12±3.01)m/s,common peroneal motor nerve conduction velocity of(49.86±2.89)m/s,median sensory nerve conduction velocity of(48.72±2.25)m/s,common peroneal sensory nerve conduction velocity of(45.86±2.56)m/s,which were higher than those of(47.12±2.96),(42.67±1.69),(42.09±1.79)and(38.12±2.61)m/s in the control group,and the differences were statistically significant(P<0.05).SOD of(134.42±6.80)U/ml in the experimental group was higher than that of(125.09±6.26)U/ml in the control group;MDA of(4.09±0.73)nmol/ml in the experimental group was lower than that of(5.88±0.60)nmol/ml in the control group;all the differences were statistically significant(P<0.05).Conclusion Mecobalamin combined withα-lipoic acid has a significant effect in the treatment of diabetic peripheral neuropathy patients,which can significantly improve the neuroelectrophysiological indexes of patients,and has a high curative effect,which can be popularized.
