中药足浴法治疗糖尿病足溃疡有效性与安全性的Meta分析

摘　　要：目的:系统评价中药足浴法治疗糖尿病足溃疡的有效性与安全性。方法:检索Pubmed、Web of Science、Cochrane Library及中国知网、维普、万方、中国生物医学文献服务平台自建库至2022年6月发表的关于中药足浴法治疗糖尿病足溃疡的临床随机对照试验。筛选所有检索文献,提取相关数据,运用Cochrane风险偏倚评估工具对纳入文献进行质量评价,采用RevMan 5.3软件进行Meta分析。结果:经筛选纳入文献38篇,总计3316例糖尿病足溃疡患者,其中中药足浴组1690例,对照组1626例,Meta分析结果示:中药足浴组治疗糖尿病足溃疡在提高治疗总有效率[优势比(Odds Ratio,OR)=4.04,95%置信区间(Confidence Interval,CI)(3.25,5.03),Z=12.55,P<0.00001],提高愈合率[OR=2.63,95%CI(2.15,3.21),Z=9.42,P<0.00001]、改善患者踝肱指数[MD=0.13,95%CI(0.06,0.20),Z=3.51,P=0.0004]、缩短溃疡愈合时间[均数差(Mean Difference,MD)=-6.42,95%CI(-8.33,-4.50),Z=6.57,P<0.00001]方面优于对照组,且无明显不良反应。结论:中药足浴法可提高临床治疗糖尿病足溃疡的整体疗效,促进创面愈合,且具有良好的安全性。Objective:To systematically evaluate the efficacy and safety of TCM medicine foot bath in treating diabetic foot ulcer.Methods:Randomized controlled trials(RCTs)on TCM medicine foot bath in treating diabetic foot were retrieved from Pubmed,Web of Science,Cochrane Library,CNKI,VIP,Wanfang Data,China Biomedical Literature Service Platform from the establishment of the database to June 2022.All literature was screened,relevant data was extracted,the quality of the included literatures was evaluated using Cochrane risk bias assessment tool,and meta-analysis was performed using RevMan 5.3 software.Results:A total of 3316 patients with diabetic foot ulcer were included in 38 studies,including 1690 patients in the TCM foot bath group and 1626 patients in the control group.The results of metaanalysis showed that in the treatment of diabetic foot ulcer in the TCM foot bath group,the total effective rate was improved[OR=4.04,95%CI(3.25,5.03),Z=12.55,P<0.00001],the healing rate was improved[OR=2.63,95%CI(2.15,3.21),Z=9.42,P<0.00001],the ankle-brachial index was ameliorated[MD=0.13,95%CI(0.06,0.20),Z=3.51,P=0.0004],and the healing time of ulcer was shortened[MD=-6.42,95%CI(-8.33,-4.50),Z=6.57,P<0.00001],which were better than those in the control group,and there was no obvious adverse reactions.Conclusion:TCM medicine foot bath can improve the overall effect on diabetic foot ulcer,it can promote wound healing,and has good safety.
