更年安坤汤联合芬吗通治疗肾阴阳两虚型围绝经期综合征的临床疗效观察

摘　　要：目的:观察更年安坤汤联合芬吗通治疗肾阴阳两虚型围绝经期综合征的临床疗效。方法:选取2021年4月―2022年4月日照市中医医院收治的60例围绝经期综合征妇女,根据就诊期间不同的药物治疗方案进行分组,分为治疗组30例(口服更年安坤汤联合芬吗通)和对照组30例(口服芬吗通),疗程3个月。观察两组患者治疗前后的Kupperman积分、中医证候积分变化及雌激素、卵泡刺激素、促黄体刺激素、骨密度、子宫内膜厚度的变化及其安全性。结果:Kupperman积分、中医证候积分组间比较差异有统计学意义(P<0.05),治疗组优于对照组;治疗后治疗组的雌二醇水平明显高于对照组,卵泡刺激素水平明显低于对照组(P<0.05);对照组的子宫内膜厚度明显厚于治疗组(P<0.05),治疗组能更好地改善子宫内膜情况且无过度增生;两组患者治疗前后腰椎(L1~L4)、髋关节骨密度差异均有统计学意义(P<0.05),在改善腰椎(L1~L4)、髋关节骨密度方面治疗组优于对照组。结论:更年安坤汤联合芬吗通在改善围绝经期综合征患者的临床症状方面疗效确切,有利于改善性激素水平,增加骨密度,减少不良反应的发生,其疗效确切且用药安全性高。Objective:To observe the clinical effect of the Gengnian Ankun decoction(更年安坤汤)plus femoston in the treatment of perimenopausal syndrome with kidney Yin and Yang(阴阳)deficiency.Methods:A total of 60 women with perimenopausal syndrome admitted to Rizhao Hospital of Traditional Chinese Medicine from April 2021 to April 2022 were selected and divided into the treatment group(30 cases)(oral administration of the Gengnian Ankun decoction plus femoston)and the control group(30 cases)(oral administration of femoston),with a course of 3 months.The changes of Kupperman score,TCM syndrome score,estrogen(E2),follicle stimulating hormone(FSH),luteinizing hormone,bone mineral density and endometrial thickness before and after treatment and their safety were observed in the two groups.Results:There were significant differences in Kupperman score and TCM syndrome score between the two groups(P<0.05),and the treatment group was better than the control group.After treatment,the E2 level in the treatment group was significantly higher than that in the control group,and the FSH level in the treatment group was significantly lower than that in the control group(P<0.05).The endometrial thickness in the control group was significantly thicker than the treatment group(P<0.05),and the treatment group could better improve the endometrial condition without excessive hyperplasia.There were significant differences in bone mineral density of lumbar spine(L1-L4)and hip joint between the two groups before and after treatment(P<0.05),and the treatment group was superior to the control group in improving bone mineral density of lumbar spine(L1-L4)and hip joint.Conclusions:The Gengnian Ankun decoction plus femoston is effective in relieving the clinical symptoms of patients with perimenopausal syndrome,which is beneficial to improve the level of sex hormones,increase bone mineral density and reduce the occurrence of adverse reactions.It has definite efficacy and high safety.
