针刺干预帕金森叠加综合征1例病案报道

摘　　要：帕金森病是一种发生率很高的神经内科疾病,最近几年发病率很高,而且发病机制十分复杂,对患者的正常生活影响很大。而帕金森叠加综合征(Parkinsonism-plus Syndrome,PPS)是一种典型的病症,其发病早期与帕金森病具有很高的相似度,但是在病理、病因及各指标上两者差异巨大,对患者预后和生活质量产生巨大影响,造成家庭及社会负担,且目前临床治疗多以西药为主,易产生不良反应。针刺治疗是一种无不良反应的传统疗法,其操作便捷、价格低廉、绿色环保,是中医学不可分割的重要组成部分。笔者通过追踪、记录1例针刺干预PPS患者的临床资料,对比、分析其病程过程、病情变化,发现该患者对多巴丝肼片早期反应较好,但因其后期治疗作用和不良反应同时产生,如幻视、幻听等,故本病案详细记录了停用多巴丝肼片采用针刺干预的治疗方法取得的临床效果,以期为针刺治疗临床优势病种及疑难病种提供借鉴意义和基础资料,并为未来证据等级较高的临床随机对照试验提供经验资料。报告显示针刺对PPS有明显的治疗效果。Parkinson’s disease is a neurological disease with a high incidence in recent years,and its pathogenesis is very complex,which has a great impact on the normal life of patients.Parkinson’s-plus syndrome(PPS)is a typical disease,and its early onset has a high similarity with Parkinson’s disease,but there are huge differences in pathology,etiology and various indicators.It has a great impact on the prognosis and quality of life of patients,causing family and social burden,and the current clinical treatment is mainly Western medicine,which is easy to produce adverse reactions.Acupuncture therapy is a kind of non-toxic and side effects of traditional therapy,its convenient operation,low price,green environmental protection makes it an integral part of TCM.By tracking and recording the clinical data of one patient with PPS who is treated by acupuncture,the author compares and analyzes the course and changes of the disease,and finds that the patient has a good early response to levodopa and benserazide hydrochloride tablets,but because of the later therapeutic effect and adverse reactions,such as hallucination,auditory hallucination,etc.,this case records in detail the clinical effect of acupuncture intervention after discontinuation of levodopa and benserazide hydrochloride tablets,so as to provide reference and basic data for acupuncture treatment of clinical predominance and refractory diseases,and to provide experience for future randomized controlled trials with higher levels of evidence.The report shows that acupuncture has obvious beneficial therapeutic effect on PPS.
