基于肠道菌群探讨化痰祛湿法治疗年龄相关性黄斑变性的机制

摘  要:年龄相关性黄斑变性(Age-related Macular Degeneration,AMD)会引起视物变形、视力下降,严重的会导致视力丧失。近年来该病已成为我国中老年人的主要致盲因素,随着中国老龄化的加速,患病人群呈现逐年增长的趋势。黄斑变性是世界眼科界公认的最难医治的眼科疾病之一,但是其具体病因、病机理论还有待继续探讨完善。有研究表明,受肠道菌群影响调控的脂质代谢通路紊乱可能是造成AMD的关键因素。根据这一关键因素进行推测,中医药所具有的有效影响因子作用于肠道菌群调控脂质循环通路的标靶点,可通过影响肠道菌群-脂质代谢-AMD通路进而产生积极的临床疗效。文章从肠道菌群理论切入,讨论化痰祛湿法改善肠道微生态、调控脂质代谢进而治疗AMD的方法,摸索理论依据,并为进一步向临床推广运用打下基础。Age-related macular degeneration can cause visual deformation,hypopsia,and blind.In recent years,the disease has become the main blindness factor of middle-aged and elderly people in China.With the acceleration of aging in China,the patient population shows a trend of increasing year by year.Macular degeneration is recognized as one of the most difficult ophthalmic diseases in the world,but its specific etiology and pathogenesis theory need to be improved.Studies have shown that disordered lipid metabolism pathway regulated by the influence of intestinal microflora may be a key factor in age-related macular degeneration.Based on this key factor,it is speculated that the effective influencing factors of TCM medicine act on the target of intestinal flora regulating lipid cycle pathway,and can produce positive clinical efficacy by affecting intestinal flora-lipid metabolism-AMD pathway.Starting from the theory of intestinal flora,this paper discusses the methods of ameliorating intestinal microecology,and regulating lipid metabolism on agerelated macular degeneration to explore the theoretical basis,and lay a foundation for its further promotion and clinical application.
