遵义市围孕期女性血清同型半胱氨酸水平及影响因素分析

摘　　要：目的:获取遵义市孕前及孕期女性血清同型半胱氨酸(homocysteine,Hcy)值并了解其相关的因素。方法:采集2019年6月至2020年4月在遵义市妇幼保健院接受孕期检查的939例样本,采用相关PCR法检测5,10-亚甲基四氢叶酸还原酶(MTHFR)C677T基因位点多态性,采用循环酶法测定Hcy浓度。结果:1)<30岁人群的Hcy水平是(6.56±3.41)μmol/L,≥30岁人群的Hcy是(6.22±3.74)μmol/L,二者无统计学差异(P>0.05)。2)Hcy水平在孕前人群中的值是(8.33±2.34)μmol/L,早孕期值是(6.97±3.77)μmol/L,中孕期值是(5.67±3.13)μmol/L,晚孕期值是(5.95±2.70)μmol/L,四组Hcy水平有统计学差异(P<0.05)。3)MTHFR C677T不同基因型中,CC基因型Hcy水平是(5.72±2.89)μmol/L,CT基因型Hcy是(6.01±2.23)μmol/L,TT基因型Hcy是(8.06±6.93)μmol/L,三组Hcy水平有统计学差异(P<0.05)。结论:围孕期女性Hcy水平与MTHFR C677T基因、年龄及孕周有关。Objective:Obtain the serum homocysteine(Hcy)value of women before and during pregnancy in Zunyi City and understand its related factors.Methods:939 samples were collected from Zunyi Maternal and Child Health Hospital during pregnancy from June 2019 to April 2020.The polymorphism of 5,10-methylenetetrahydrofolate reductase(MTHFR)C677T gene locus was detected by correlation PCR,and the concentration of Hcy was measured by circulating enzyme method.Results:1)The level of Hcy in people younger than 30 years old was(6.56±3.41)μmol/L,and that in people older than 30 years old was(6.22±3.74)μmol/L,there was no statistical difference between the two groups(P>0.05).2)The value of Hcy level in the pre-pregnancy population is(8.33±2.34)μmol/L,the value of early pregnancy is(6.97±3.77)μmol/L,the value of middle pregnancy is(5.67±3.13)μmol/L,and the value of late pregnancy is(5.95±2.70)μmol/L,the level of Hcy in the four groups was significantly different(P<0.05).3)Among the different genotypes of MTHFR C677T,the level of Hcy of CC genotype is(5.72±2.89)μmol/L,that of CT genotype is(6.01±2.23)μmol/L,and that of TT genotype is(8.06±6.93)μmol/L,the level of Hcy in the three groups was statistically different(P<0.05).Conclusion:The level of Hcy in women during pregnancy is related to MTHFR C677T gene,age and gestational age.
