第二产程体位干预联合手法按摩及胎头位置异常手法矫正对初产妇分娩结局的改善观察

摘　　要：目的:观察第二产程体位干预联合手法按摩及胎头位置异常手法矫正对产妇分娩结局的改善效果。方法:回顾性分析2020年1月至2022年10月于本院妇产科经阴道自然分娩(未实施无痛分娩)的68名初产妇临床资料,依据分娩护理方法分为两组各34名。对照组实施传统分娩护理,观察组实施第二产程体位干预联合手法按摩及胎头位置异常矫正。统计两组产妇的分娩结局、第二产程时间、新生儿Apgar评分、助产护理服务总满意率并进行对比。结果:观察组的分娩结局优于对照组(P<0.05),第二产程时间短于对照组(P<0.05),新生儿Apgar评分、助产护理服务总满意率高于对照组(P<0.05)。结论:第二产程体位干预联合手法按摩及胎头位置异常矫正能够提升产妇分娩结局改善效果,融洽护患关系,值得临床使用。Objective:To observe the effect of body position intervention in the second stage of labor combined with manual massage and manual correction of abnormal fetal head position on the outcome of delivery.Methods:The clinical data of 68 primipara who were delivered naturally via vagina(without painless delivery)in our hospital from January 2020 to October 2022 were retrospectively analyzed.They were divided into two groups,34 in each group,according to the delivery nursing method.The control group received tradit ional delivery nursing,while the observation group received body position intervention in the second stage of labor combined with manual massage and correction of abnormal fetal head position.The delivery outcome,the second stage of labor,the Apgar score of newborns,and the total satisfaction rate of midwifery nursing service of the two groups were statist ically analyzed and compared.Results:The delivery outcome of the observation group was better than that of the control group(P<0.05),the second stage of labor was shorter than that of the control group(P<0.05),and the neonatal Apgar score and total satisfaction rate of midwifery care services were higher than that of the control group(P<0.05).Conclusion:Posture intervention in the second stage of labor combined with manual massage and correction of abnormal fetal head position can improve the effect of maternal delivery outcome and harmonize the nurse-patient relationship,which is worth applying.
