摘  要:目的探究肺部局部晚期放疗中应用还原型谷胱甘肽对放射性肺炎的影响。方法采取随机数表法将南通大学附属如皋医院2017年1月—2020年12月收治的80例晚期肺癌患者分为两组,一组采取单纯调强适形或三维适形放疗,另一组在上述基础上采取还原型谷胱甘肽治疗,前者为对照组,后者为研究组。观察两组不良反应发生率、放射性肺炎发生时间及发生率及相关炎症介质水平变化情况。结果研究组不良反应发生率为15.00%,低于对照组40.00%,差异有统计学意义(χ^(2)=6.270,P<0.05)。研究组放射性肺炎发生率为10.00%,低于对照组30.00%,差异有统计学意义(χ^(2)=5.000,P<0.05)。研究组放射性肺炎发生时间为(76.37±9.56)d,晚于对照组,差异有统计学意义(t=14.781,P<0.001)。治疗后,研究组白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、转化生长因子-β(TGF-β)水平为(61.22±10.14)、(52.48±9.49)、(3996.75±246.73)pg/mL,低于对照组,差异有统计学意义(t=5.221、8.522、11.137,P<0.05);研究组血管紧张素转化酶(ACE)水平为(774.36±135.29)pg/mL,高于对照组,差异有统计学意义(t=6.832,P<0.001)。结论对接受放疗的晚期局部肺癌患者应用还原型谷胱甘肽可改善毒副作用,减轻炎症反应,降低放射性肺炎发生率,延迟放射性肺炎发生时间,值得大力推广和引用。Objective To investigate the effect of reduced glutathione(GSH)on radiation pneumonia in local advanced lung radiotherapy.Methods Eighty patients with advanced lung cancer admitted to Rugao Hospital Affiliated to Nantong University from January 2017 to December 2020 were divided into two groups by random number table method.One group received pure intensity modulated conformal radiotherapy or three-dimensional conformal radiotherapy;the other group received reduced glutathione on the basis of above treatment.The former group was the control group and the latter was the study group.The aAdverse reaction rate,the occurrence time and incidence of radiation pneumonia and the changes of the levels of related inflammatory mediators were observed in the two groups.Results The incidence of adverse reactions in the study group was 15.00% lower than that in the control group 40.00%,and the difference was statistically significant(χ^(2)=6.270,P<0.05).The incidence of radiation pneumonia in the study group was 10.00%,which was lower than that in the control group 30.00%,the difference was statistically significant(χ^(2)=5.000,P<0.05).The onset time of radiation pneumonia in the study group was(76.37±9.56)d,which was later than that in the control group,the difference was statistically significant(t=14.781,P<0.001).After treatment,the levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and transforming growth factor-β(TGF-β)in the study group were(61.22±10.14)pg/mL,(52.48±9.49)pg/mLand(3996.75±246.73)pg/mL,which were lower than those in the control group,the difference was statistically significant(t=5.221,8.522,11.137,P<0.05);the level of angiotensin converting enzyme(ACE)in the study group was(774.36±135.29)pg/mL,higher than that in the control group,and the difference was statistically significant(t=6.832,P<0.001).Conclusion The application of reduced glutathione to patients with advanced local lung cancer receiving radiotherapy can improve the toxic and side effects,reduce the inflammatory reaction,redu
