夜间低氧血症对高血压患者心率变异性及心律失常的影响研究

摘　　要：研究了夜间低氧血症与高血压患者心率变异性、心律失常及血压昼夜节律之间的相关性。选取78例确诊为高血压的患者为研究对象,均对其进行睡眠监测检查,根据患者是否合并夜间低氧血症将其分为单纯组及合并组,完善超声心电图、动态血压及动态心电图检测。结果显示,合并组患者与单纯组相比较,其MSaO_(2)和LSaO_(2)指标显著降低,REI及氧减最大持续时间则明显升高,且心率变异性指标中的SDANN及rMSSD均下降,两组比较差异均具有统计学意义(P<0.05);合并组患者心律失常发生率相比较明显高于单纯组,差异有统计学意义(P<0.05)。研究发现,夜间低氧血症是引起高血压患者心率变异性及心律失常发生率增加的重要影响因素之一,且与血压、血压节律改变有一定的关系。The relationship between nocturnal hypoxemia and heart rate variability arrhythmia and circadian rhythm of blood pressure in patients with hypertension was studied.78 patients diagnosed as hypertension were selected as the research object,and all of them were monitored for sleep.According to whether the patients were combined with nocturnal hypoxemia,they were di-vided into simple group and combined group,and improved the detection of ultrasonic electrocardiogram,ambulatory blood pres-sure and ambulatory electrocardiogram.Compared with the simple group,the combined group of patients showed a significant decrease in their MSaO_(2)and LSaO_(2)indicators,while the REI and maximum duration of oxygen reduction increased significantly.Additionally,the SDANN and rMSSD in heart rate variability indicators decreased,and the differences between the two groups were statistically significant(P<0.05).The incidence of arrhythmia in the combined group was significantly higher than that in the simple group,and the diference was satstically significant(P<0.05).It is found that nocturmal hypoxemia is one of the important ctors that cause the increase of heart rate variability and arrhythmia incidence in patients with hypertension,and has a certain rela-tionship with the changes of blood pressure and blood pressure rhythm.
