中孕早期胎儿心脏超声检查在先天性心脏病筛查中的价值分析

摘　　要：分析了中孕早期胎儿心脏超声检查在先天性心脏病筛查中的价值。选择2019年1月-2023年1月在甘肃医学院附属医院就诊的948例中孕早期孕妇及胎儿(胎儿先天性心脏病高风险)作为研究样本。检查结果显示,中孕早期心脏超声筛查出房间隔缺损3例(0.32%)、室间隔缺损4例、室缺合并房缺2例、右位心2例、动脉导管粗大2例、法乐氏四联.症3例、其他5例,总检出例数为21,总检出率为2.22%;产后心脏超声检查筛查出房间隔缺损5例、室间隔缺损5例、室缺合并房缺3例、右位心2例、动脉导管粗大2例、法洛氏四联症3例、其他6例,总检出例数为26,总检出率为2.74%;产后超.声检查(金标准)检出阳性26例、阴性922例;中孕早期胎儿心脏超声检查检出阳性21例、阴性927例,准确度、敏感度、特异度分别为99.26%、76.92%、99.89%。研究表明,中孕早期胎儿心脏超声检查能准确筛查出胎儿先天性心脏病,建议临床推广。Analyzed the value of fetal cardiac ultrasound examination in the screening of congenital heart disease in early pregnancy.948 pregnant women and fetuses(with a high risk of congenital heart disease in fetuses)who visited the Affiliated Hospital of Gansu Medical College from September 2019 to March 2023 were selected as research samples.The results showed that 3 cases of atrial septal defect(0.32%),4 cases of interventricular septum defect,2 cases of ventricular septal defect with atrial septal defect,2 cases of dextrocardia,2 cases of thick ductus arteriosus,3 cases of tetralogy of Fallot,and 5 other cases were screened by cardiac ultrasound in the first trimester of pregnancy.The total number of cases detected was 21,and the total detection rate was 2.22%;After delivery,5 cases of atrial septal defect,5 cases of interventricular septum defect,3 cases of ventricular septal defect with atrial defect,2 cases of dextrocardia,2 cases of thick ductus arteriosus,3 cases of tetralogy of Fallot,and 6 other cases were screened by echocardiography.The total number of cases detected was 26,and the total detection rate was 2.74%;Postpartum ultrasound examination(gold standard)detected positive in 26 cases and negative in 922 cases;Early fetal echocardiography detected 21 cases of positive and 927 cases of negative results,with accuracy,sensitivity,and specificity of 99.26%,76.92%,and 99.89%,respectively.Research has shown that fetal echocardiography in the early stages of mid pregnancy can accurately screen for congenital heart disease in fetuses,and it is recommended to promote it in clinical practice.
