通络润目汤治疗蒸发过强型干眼的临床研究

摘　　要：目的观察通络润目汤治疗蒸发过强型干眼临床效果。方法将符合蒸发过强型干眼的138例(138只眼)患者随机分为2组,治疗组70例(70只眼)采用通络润目汤联合玻璃酸钠滴眼液治疗,对照组68例(68只眼)采用玻璃酸钠滴眼液治疗。治疗1个月,观察2组患者眼表疾病指数(OSDI)、泪膜破裂时间(BUT)、基础泪液分泌试验Ⅰ(SⅠT)等疗效指标。结果(1)OSDI评分:治疗前后比较,治疗组差异有统计学意义(t=34.843,P=0.000),而对照组差异无统计学意义(P>0.05)。治疗后2组间比较,治疗组患者OSDI评分优于对照组,差异有统计学意义(t=-11.664,P=0.000)。(2)BUT:治疗前后比较,治疗组差异有统计学意义(t=-11.830,P=0.000),而对照组差异无统计学意义(P>0.05)。治疗后2组间比较,治疗组优于对照组,差异有统计学意义(t=9.800,P=0.000)。(3)SⅠT:治疗前后比较,治疗组差异有统计学意义(t=-8.518,P=0.000),而对照组差异无统计学意义(P>0.05)。治疗后2组间比较,治疗组优于对照组,差异有统计学意义(t=9.648,P=0.000)。(4)临床疗效:治疗组临床总有效率高于对照组,差异有统计学意义(χ^(2)=4.161,P=0.041)。结论通络润目汤联合玻璃酸钠滴眼液能够促进泪液分泌,改善泪膜功能,能够改善蒸发过强型干眼症状。OBJECTIVE To observe the clinical effects of Tongluo Runmu Decoction in the treatment of evaporative dry eye.METHODS A total of 138 patients(138 eyes)with evaporative dry eye were randomly divided into two groups.The treatment group(70 patients,70 eyes)received Tongluo Runmu Decoction combined with sodium hyaluronate eye drops,while the control group(68 patients,68 eyes)received only sodium hyaluronate eye drops.The efficacy was evaluated after one month of treatment based on ocular surface disease index(OSDI),tear film break-up time(BUT),and SchirmerⅠtest(SⅠT)as indicators.RESULTS(1)OSDI scores:There was a significant difference in the treatment group before and after treatment(t=34.843,P=0.000),while there was no significant difference in the control group(P>0.05).After treatment,the OSDI scores of the treatment group were significantly better than those of the control group(t=-11.664,P=0.000).(2)BUT:There was a significant difference in the treatment group before and after treatment(t=-11.830,P=0.000),while there was no significant difference in the control group(P>0.05).After treatment,BUT of the treatment group was significantly better than that of the control group(t=9.800,P=0.000).(3)SⅠT:There was a significant difference in the treatment group before and after treatment(t=-8.518,P=0.000),while there was no significant difference in the control group(P>0.05).After treatment,SⅠT of the treatment group was significantly better than that of the control group(t=9.648,P=0.000).(4)Clinical efficacy:The total effective rate of the treatment group was higher than that of the control group,and the difference was statistically significant(χ^(2)=4.161,P=0.041).CONCLUSIONS Tongluo Runmu Decoction combined with sodium hyaluronate eye drops can promote tear secretion,improve tear film function,and alleviate symptoms of evaporative dry eye.
