参苓白术散对MODSE合并营养不良患者营养状况及炎症因子的影响

摘　　要：目的 探讨参苓白术散对老年多器官功能不全综合征(MODSE)合并营养不良患者营养状况及炎症因子的影响。方法 选择符合西医MODSE合并营养不良诊断且中医辨证为脾胃虚弱证的80例患者,采用随机数字表法分为观察组、对照组各40例。2组患者入院后均按西医常规对症支持处理,观察组在以上基础上,予以参苓白术散口服或鼻饲,观察2组治疗前后中医证候积分、ALB、Hb、hs-CRP、TNF-α、IL-6、IL-10。结果 2组患者治疗后脾胃虚弱证型评分、hs-CRP、TNF-α、IL-6、IL-10水平较治疗前均明显降低,差异均具有统计学意义(P<0.05);ALB、Hb水平较治疗前升高,差异均具有统计学意义(P<0.05)。结论 参苓白术散可以降低MODSE患者脾胃虚弱证型评分,改善患者营养不良水平;参苓白术散能够有效降低炎症因子在MODSE发病过程中的表达,从而缓解临床病症及延缓病情的进展。Objective To investigate the effect of Shenling Baizhu powder on nutritional status and inflammatory factors in MODSE patients with malnutrition.Methods A total of 80 patients with MODSE combined with malnutrition diagnosed by western medicine and syndrome differentiation of spleen and stomach weakness were randomly divided into observation group and control group by digital random method,with 40 cases in each group.On the basis of the above treatment,the observation group was given Shenling Baizhu powder orally or nasal feeding.The TCM syndrome score,ALB,Hb,hs⁃CRP,TNF⁃α,IL⁃6 and IL⁃10 of the two groups were observed before and after treatment.Results After treatment,spleen and stomach weakness syndrome score,Hs⁃CRP,TNF⁃α,IL⁃6 and IL⁃10 levels in 2 groups were significantly decreased compared with before treatment,and the difference was statistical significance(P<0.05),and ALB and Hb levels were increased compared with before treatment,and the difference was statistical significance(P<0.05).Conclusion Shenling Baizhu powder can reduce the score of spleen stomach asthenia syndrome in MODSE patients and improve the level of malnutrition.Shenling Baizhu powder can effectively reduce the expression of inflammatory factors in the pathogenesis of MODSE,so as to alleviate clinical symptoms and delay the progress of the disease.
