针药序贯疗法治疗急性腰扭伤临床研究

摘　　要：目的对比针灸、中药塌渍及序贯疗法治疗急性腰扭伤的临床疗效。方法将128例诊断为急性腰扭伤的患者随机分组,每组32例,其中针灸组给予肾俞、气海俞、大肠俞、关元俞、腰阳关、腰部夹脊穴、委中、承筋和承山针刺治疗,塌渍组给予中药塌渍疗法,序贯组按照预设治疗顺序给予针灸、中药塌渍及红外线照射治疗,对照组给予红外线照射治疗;治疗7 d后评价所有组别患者症状积分、腰疼视觉模拟量表(VAS)评分、腰部活动ROM评分,并评价所有患者治疗有效率。结果各组患者治疗后症状均有所缓解,其中序贯组患者症状积分、腰疼视觉模拟量表(VAS)评分、腰部活动ROM评分均显著低于对照组(P<0.05);序贯组的治疗有效率高于针灸组、塌渍组及对照组(P<0.05)。结论针灸、塌渍及红外线治疗的序贯疗法,在治疗急性腰扭伤患者过程中具有更好的疗效,优于其他单独疗法的治疗方式。Objective To compare the clinical effect of acupuncture,Chinese medicine application and sequential therapies in the treatment of acute lumbar sprains.Methods 128 patients diagnosed with acute lumbar sprain were randomly divided into two groups,with 32 cases in each group.The acupuncture group was given Shenshu(BL23),Qihaishu(BL24),Dachangshu(BL25),Guanyuanshu(BL26),Yaoyangguan(DU3),Lumbar Jiaji,acupuncture treatment for Weizhong(BL40),Chengjin(BL56)and Chengshan(BL57).The application group was given Chinese medicine application therapy,and the sequential group was given acupuncture,Chinese medicine application and infrared radiation according to the preset treatment sequence,and the control group was given infrared radiation treatment.7 days after treatment,the symptom score,the visual analog scale(VAS)score of back pain,and the ROM score of waist activity were evaluated for all groups of patients,and the treatment efficiency of all patients was evaluated.Results Symptoms were relieved in all groups after treatment,and symptom scores,visual analog scale(VAS)scores and ROM scores in sequential group were significantly lower than those in control group(P<0.05).The effective rate of the sequential group was higher than that of the acupuncture group(P<0.05).Conclusion The sequential therapies of acupuncture and moxibustion,application and infrared treatment have better curative effects in the treatment of acute lumbar sprain,and are superior to other treatment methods alone.
