闽东地区畲族人群的ApoE基因多态性与他汀类药物疗效的相关性研究

摘　　要：目的:探讨闽东地区畲族人群ApoE基因多态性以及畲族高脂血症患者的ApoE基因型与阿托伐他汀钙降脂效果的相关性。方法:采用药物基因床旁快检系统检测50例健康汉族受检者和50例健康畲族受检者的ApoE基因多态性。40名畲族高脂血症患者和40名汉族高脂血症患者口服阿托伐他汀20 mg·d^(-1)治疗,通过检测用药前以及用药8周后的低密度脂蛋白胆固醇(LDL-C)水平,评价降脂疗效。结果:50名汉族健康受检者ApoE基因型分布为E2/E3型8.0%,E3/E3型66.0%,E3/E4型24.0%,E4/E4型1.0%。50名畲族健康受检者基因型分布为:E2/E3型10.0%,E3/E3型86.0%,E3/E4型4.0%,E4/E4型0%。两族E3、E4等位基因,E3/E3基因型和E3/E4基因型频率差异有统计学意义(P<0.05)。阿托伐他汀治疗8周后,汉族及畲族E3/E3基因型组高脂血症患者LDL-C水平变化率均大于E3/E4+E4/E4基因型组,差异均有统计学意义(P<0.05)。结论:闽东地区畲族人群E4等位基因及E3/E4型频率低于汉族;高脂血症患者的ApoE基因型与阿托伐他汀钙的降脂效果有相关性,E4基因携带者的降脂效果差。Objective:To investigate the ApoE gene polymorphisms of She nationality population,and the correlation between ApoE genetype and the cholesterol-lowering effect of atorvastatin calcium in hyperlipidemia patients of She nationality in eastern Fujian.Methods:The point-of-care testing system of drug-related genes was adopted to detect the ApoE gene polymorphisms of 50 health Han nationality people and 50 health She nationality people.40 hyperlipidemia patients of Han nationality and 40 hyperlipidemia patients of She nationality received atorvastatin calcium at a oral dose of 20 mg·d^(-1).The lipid-lowering efficacy was evaluated through detecting serum low-density lipoprotein cholesterol(LDL-C)level before and 8 weeks after the treatment.Results:The gene distribution of ApoE gene was 8.0%,66.0%,24.0%and 1.0%respectively for E2/E3,E3/E3,E3/E4 and E4/E4 group in 50 health Han nationality people.The gene distribution of ApoE gene was 10.0%,86.0%,4.0%and 0%respectively for E2/E3,E3/E3,E3/E4 and E4/E4 group in 50 health She nationality people.The frequencies of E3,E4 allele gene and E3/E3,E3/E4 genotype in two nationalities showed statistical differences(P<0.05).After receiving atorvastatin for 8 weeks,the decreased LDL-C levels showed statistical differences(P<0.05)among E3/E3 and E3/E4+E4/E4 groups in two nationalities.Conclusion:The frequencies of E4 allele gene and E3/E4 genotype in She nationality in eastern Fujian is less than the frequencies in Han nationality.The gene polymorphism of ApoE is correlated with efficacy of atorvastatin calcium in hyperlipidemia patients of She nationality.The lipid-lowering efficacy of E4 gene carrier is poor.
