摘　　要：目的评价机器人辅助腹腔镜前列腺癌根治术(robot-assisted laparoscopic radical prostatectomy,RLRP)治疗大体积前列腺癌的预后效果。方法随机选择2018年6月—2020年1月赤峰市医院泌尿外科收治的大体积前列腺癌疾病患者80例为研究对象。根据患者意愿,分为采取RLRP治疗的观察组、腹腔镜前列腺癌根治术(laparoscopic radical prostatectomy,LRP)治疗的对照组,各40例。比较两组患者的手术治疗情况,包括术中出血量,手术以及术后下床活动、引流管拔除、肛门排气、住院时间等治疗相关指标,并发症以及随访6个月生化复发情况。结果观察组术后并发症发生率、生化复发率均低于对照组,差异有统计学意义(P<0.05);观察组术中出血量、手术时间、下床活动时间、引流管拔除时间、肛门排气时间、住院时间均少于对照组,差异有统计学意义(P<0.05)。结论与LRP术式比较,RLRP治疗大体积前列腺癌能进一步改善患者的预后,提升患者的安全性,是有效治疗术式。Objective Evaluate the prognostic effect of robot assisted laparoscopic radical prostatectomy(RLRP)in the treatment of large volume prostate cancer.Methods 80 patients with large volume prostate cancer admitted to the urology department of Chifeng City Hospital from June 2018 to January 2020 were randomly selected as the study subjects.According to the patient's wishes,they were divided into an observation group receiving RLRP treatment and a control group receiving laparoscopic radical prostatectomy(LRP)treatment,with 40 patients in each group.Compared the surgical treatment status of two groups of patients,including intraoperative bleeding volume,surgical and postoperative bed mobility,drainage tube removal,anal exhaust,hospital stay,and other treatment related indicators,complications,and biochemical recurrence at a follow-up of 6 months.Results The incidence of postoperative complications and biochemical recurrence in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The observation group had less intraoperative bleeding,surgery time,bedridden activity time,drainage tube removal time,anal exhaust time,and hospital stay than the control group,the difference was statistically significant differences(P<0.05).Conclusion Compared with LRP surgery,RLRP treatment for large volume prostate cancer further improves the patient's prognosis,enhances patient safety,and is an effective treatment procedure.
