分析白内障超声乳化联合房角分离术治疗原发性闭角型青光眼合并白内障的临床效果

摘　　要：目的分析原发性闭角型青光眼合并白内障疾病特点、治疗要点,评价白内障超声乳化联合房角分离术治疗的临床优势。方法简单随机选择在2019年3月—2020年5月期间赤峰市医院眼科收治的原发性闭角型青光眼合并白内障疾病患者80例作为研究对象。依据随机数表法分组,每组各40例,对照组采取超声乳化+小梁切除术治疗,观察组采取白内障超声乳化+房角分离术治疗。比较两组临床疗效。结果组间治疗效果与并发症情况(角膜水肿、黄斑水肿等)比较,观察组治疗总有效率为95.00%高于对照组的75.00%,并发症发生率为5.00%低于对照组的25.00%,差异有统计学意义(χ^(2)=6.275、6.275,P<0.05)。术前组间视力、眼压、中央前房深度接近,差异无统计学意义(P>0.05),术后6个月比较观察组优于对照组,差异有统计学意义(P<0.05);观察组患者术后6个月生活质量评分明显高于对照组,差异有统计学意义(P<0.05)。结论原发性闭角型青光眼合并白内障发生情况下,直接影响患者视力、基本生活状况,需尽早诊断与治疗。白内障超声乳化+房角分离术治疗原发性闭角型青光眼并白内障具有安全性、有效性价值,促进患者视力、眼压等改善,提升了患者的生活质量,是有效手术治疗方案。Objective To analyze the characteristics and treatment points of primary angle-closure glaucoma combined with cataract,and to evaluate the clinical advantages of phacoemulsification combined with atrial angle separation.Methods 80 patients with primary angle-closure glaucoma complicated with cataract treated in the Ophthalmology department of Chifeng Hospital from March 2019 to May 2020 were simple randomly selected as the study objects.The patients in each group were divided into 40 cases according to random number table method.The control group was treated with phacoemulsification+trabeculectomy,and the observation group was treated with phacoemulsification+atrial Angle separation.The clinical efficacy of the two groups was compared.Results Comparing the treatment effect and complications(corneal edema,macular edema,etc.)between the groups,the total effective rate of the observation group was 95.00% higher than that of the control group was 75.00%,and the complication rate was 5.00% lower than that of the control group(25.00%),the difference was statistically significant(χ^(2)=6.275,6.275,P<0.05).Preoperative visual acuity,intraocular pressure and central anterior chamber depth were close to each other,and the difference was not statistically significant(P>0.05).6 months after surgery,observation group was better than control group,and the difference was statistically significant(P<0.05).Life quality score of observation group was significantly higher than control group 6 months after operation,the difference was statistically significant(P<0.05).Conclusion In the case of primary angle-closure glaucoma combined with cataract,it directly affects the visual acuity and basic living conditions of patients,and requires early diagnosis and treatment.Phacoemulsification and angle separation of cataract in the treatment of primary angle-closure glaucoma combined with cataract has safety and effectiveness value,promotes the improvement of visual acuity and intraocular pressure,and improves the quality of life of patien
