独活寄生汤治疗对慢性创伤性膝关节滑膜炎患者治疗依从性及临床症状评分的影响

摘　　要：目的 分析慢性创伤性膝关节滑膜炎患者应用独活寄生汤治疗对治疗依从性及临床症状评分的影响。方法 随机选取2017年1月—2021年12月漳州正兴医院收治的慢性创伤性膝关节滑膜炎患者60例,根据不同治疗方法将患者分为研究组(30例)、对照组(30例)。对照组采用常规方案治疗,研究组在对照组治疗基础上采用独活寄生汤治疗。治疗4周,比较两组总有效率、治疗依从性、美国特种外科医院膝关节功能量表(HSS)评分、临床症状评分。结果 研究组总有效率(96.67%)高于对照组(73.33%),差异有统计学意义(χ^(2)=4.706,P<0.05)。研究组治疗依从度(93.33%)高于对照组(70.00%),差异有统计学意义(χ^(2)=4.007,P<0.05)。研究组HSS评分(89.1±4.8)分高于对照组(82.2±4.1)分,且研究组白细胞介素1β(IL-1β)为(6.6±1.3)ng/L、肿瘤坏死因子α(TNF-α)为(7.3±1.2)ng/L,均低于对照组(8.7±1.9)ng/L、(8.9±1.8)ng/L,差异有统计学意义(t=5.987、4.996、4.051,P<0.05)。研究组各项临床症状评分均低于对照组,差异有统计学意义(P<0.05)。结论 独活寄生汤具有祛湿除痹、补益肝肾、行气止痛、强筋健骨的功效,可减轻慢性创伤性膝关节滑膜炎患者膝关节肿痛、屈伸不利等症状,提高患者的治疗依从性。Objective To analyze the effect of Duhuo Jisheng Decoction on treatment compliance and clinical symptom score of chronic traumatic knee synovitis.Methods Sixty patients with chronic traumatic synovitis of knee joint admitted to Zhangzhou Zhengxing Hospital from January 2017 to December 2021 were randomly selected and divided into study group(30 cases)and control group(30 cases)according to different treatment methods.The control group was treated with conventional regimen,and the study group was treated with Duhuo Jisheng Decoction on the basis of the control group.After 4 weeks of treatment,the total effective rate,treatment compliance,Hospital for Special Surgery(HSS)score and clinical symptom score of the two groups were compared.Results The total effective rate of the study group(96.67%)was higher than that of the control group(73.33%),and the difference was statistically significant(χ^(2)=4.706,P<0.05).The treatment compliance of the study group(93.33%)was higher than that of the control group(70.00%),and the difference was statistically significant(χ^(2)=4.007,P<0.05).The HSS score of the study group(89.1±4.8)points was higher than that of the control group(82.2±4.1)points,and the levels of interleukin-1β(IL-1β)(6.6±1.3)ng/L and tumor necrosis factor-α(TNF-α)(7.3±1.2)ng/L were lower than those of the control group(8.7±1.9)ng/L and(8.9±1.8)ng/L,and the difference was statistically significant(t=5.987,4.996,4.051,P<0.05).The scores of clinical symptoms in the study group were lower than those in the control group, and the difference was statistically significant (P<0.05). Conclusion Duhuo Jisheng Decoction has the effects of dispelling dampness and removingbi, tonifying liver and kidney, relieving qi and relieving pain, strengthening tendons and bones, and can reduce thesymptoms of knee swelling pain and adverse flexion and extension in patients with chronic traumatic synovitis of kneejoint, and improve the treatment compliance of patients.
