肾镜下经尿道气压弹道-超声碎石清石术治疗下尿路结石的疗效

摘　　要：目的 分析选用肾镜下经尿道气压弹道-超声碎石清石术治疗下尿路结石患者的真实功效。方法 便利选择中国人民解放军陆军第七十三集团军医院2012年5月—2019年5月收治的下尿路结石患者120例,随机分为观察组和对照组,每组60例。对照组选择经尿道肾镜气压弹道碎石术,观察组选择经尿道肾镜气压弹道-超声碎石术。比较两组一次结石清除率、手术相关指标、生存质量和不良反应情况。结果 观察组各种手术指标时间短于对照组,差异有统计学意义(P<0.05);观察组一次结石清除率100.00%大于对照组70.00%,差异有统计学意义(χ^(2)=21.176,P<0.05);观察组总体生存质量要优于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率1.67%低于对照组13.33%,差异有统计学意义(P<0.05)。结论 肾镜下经尿道气压弹道-超声碎石清石治疗下尿路结石疗效显著,能有效提高一次结石清除率,缩短术中结石清除时间,提高患者生存质量,且安全性好,并发症风险低。Objective To analyze the real efficacy of nephroscopic transurethral pressure ballistic-ultrasonic lithotripsy for patients with lower urinary tract calculi.Methods A total of 120 patients with lower urinary tract calculi admitted to Military Hospital of the 73rd Army Group of the Chinese People's Liberation Army from May 2012 to May 2019 were conveniently selected and randomly divided into observation group and control group,60 cases in each group.The control group chose transurethral nephroscopic pneumatic lithotripsy,and the observation group chose transurethral nephroscopic pneumatic lithotripsy-ultrasonic lithotripsy.The primary stone-free rate,surgery-related indicators,quality of life and adverse reactions were compared between the two groups.Results Time of various surgical indexes in the observation group were shorter than those in the control group,and the difference was statistically significant(P<0.05);the one-time stone clearance rate of the observation group 100.00%was higher than that of the control group of 70.00%,and the difference was statistically significant(χ^(2)=21.176,P<0.05);the overall quality of life of the observation group was better than that of the control group,the difference was statistically significant(P<0.05);the incidence of adverse reactions in the observation group 1.67%was lower than that of the control group 13.33%,the difference was statistically significant(P<0.05).Conclusion Nephroscopic transurethral pressure ballisticultrasonic lithotripsy is effective in the treatment of lower urinary tract calculi.It can effectively improve the stone-free rate at one time,shorten the stone-free time during the operation,improve the quality of life of patients,and has good safety and low risk of complications.
