单髁膝关节置换与全膝关节置换治疗膝骨关节炎的疗效比较

摘　　要：目的比较单髁膝关节置换(UKA)和全膝关节置换(TKA)治疗膝骨关节炎(KOA)的疗效。方法将50例KOA患者按照手术方式不同分为UKA组(采用UKA治疗,25例)和TKA组(采用TKA治疗,25例)。记录两组术中出血量、并发症发生情况、膝关节活动度、疼痛缓解情况。采用HSS评分评价膝关节功能。结果患者均获得12个月随访。两组术后3、12个月的膝关节活动度及术后12个月的HSS评分均明显优于术前(P<0.05)。术中出血量及术后3、12个月的膝关节活动度UKA组均明显优于TKA组(P<0.05)。术后并发症发生率及术后12个月的HSS评分、疼痛缓解优良率两组比较差异均无统计学意义(P>0.05)。结论采用UKA和TKA治疗KOA临床疗效均满意,但UKA在术中出血量、术后膝关节活动度方面更具优势。Objective To compare the efficacy of unicompartmental knee arthroplasty(UKA)and total knee arthroplasty(TKA)in the treatment of knee osteoarthritis(KOA).Methods The 50 patients with KOA were divided into UKA group(25 cases were treated with UKA)and TKA group(25 cases were treated with TKA).The intraoperative bleeding volume,complications incidence situation,knee range of motion(ROM)and pain relief condition were recorded.HSS score was used to assess the knee joint function.Results All patients were followed up for 12 months.Knee ROM at 3,12 months postoperation and HSS score of 12 months postoperation were significantly better than the preoperation(P<0.05).The intraoperative blood loss,knee ROM at 3,12 months postoperation in UKA group were significantly better than those in TKA group(P<0.05).There were no significant differences in the incidence rate of postoperative complications,HSS score at postoperative 12 months and the excellent-good rate of pain relief between two groups(P>0.05).Conclusions Both UKA and TKA are effective in the treatment of KOA,while UKA has more advantages in the terms of intraoperative blood loss,postoperative knee ROM.
