摘 要:目的比较不同理疗方法治疗延迟性肌肉酸痛的疗效,为最佳治疗方式的选择提供参考依据。方法计算机检索中国知网、万方数据库、维普数据库、PubMed、Web of Science、EMbase、The Cochrane Library搜集不同理疗方法治疗延迟性肌肉酸痛的随机对照试验类文章,检索时限均从建库至2022年5月20日。运用End Note X9.1筛选文献,采用Cochrane风险偏倚评估工具对纳入的文献进行质量评价,采用Stata 16.0进行网状Meta分析。结果共纳入28项随机对照实验,包括836例延迟性肌肉酸痛患者。网状Meta分析结果显示:1)在减轻肌肉疼痛方面,电磁场疗法(SMD=-1.09,95%CI[-1.93,-0.24],p=0.012)、光疗(SMD=-1.02,95%CI[-1.64,-0.39],p=0.001)、电刺激(SMD=-0.79,95%CI[-1.26,-0.31],p=0.001)、超声波(SMD=-0.70,95%CI[-1.16,-0.25],p=0.003)、冷疗(SMD=-0.63,95%CI[-0.86,-0.40],p<0.0001)均优于对照组;排序结果显示:电磁场疗法(82.5%)>光疗(82.4%)>电刺激(67.9%)>超声波(60.8%)>冷疗(54.6%);2)在降低血清肌酸激酶方面,光疗(SMD=-3.01,95%CI[-3.67,-2.35],p<0.0001)、超声波[SMD=-1.34,95%CI[-2.15,-0.54],p=0.001]、冷疗(SMD=-0.17,95%CI[-0.30,-0.03],p=0.018)均优于对照组;排序结果显示:光疗(100.0%)>超声波(83.1%)>冷疗(53.6%)。结论在减轻肌肉疼痛方面,电磁场疗法的效果最佳,其次为光疗、电刺激、超声波、冷疗;在降低血清肌酸激酶方面,光疗效果最佳,其次为超声波、冷疗。Objective A network meta-analysis was performed to compare the efficacy of different physiotherapy methods on delayed onset muscle soreness,so as to provide a reference basis for selecting the best treatment modality.Methods The CNKI,WanFang Data,VIP databases,PubMed,Web of Science,EMbase,The Cochrane Library were electronically searched to collect the articles of randomized controlled trials of different physiotherapy methods for delayed onset muscle soreness,from inception to May 20,2022.EndNote X9.1 was used to screen the literature.The Cochrane Risk Bias Assessment Tool was used to evaluate the quality of the included literature.Network meta-analysis was performed with Stata 16.0.Results A total of 28 randomized controlled trials involving 836 delayed onset muscle soreness patients were included.The results of network meta-analysis showed that:1)In easing muscle pain,electromagnetic field therapy(SMD=-1.09,95%CI[-1.93,-0.24],p=0.012),phototherapy(SMD=-1.02,95%CI[-1.64,-0.39],p=0.001),electrostimulation(SMD=-0.79,95%CI[-1.26,-0.31],p=0.001),ultrasound(SMD=-0.70,95%CI[-1.16,-0.25],p=0.003),cold therapy(SMD=-0.63,95%CI[-0.86,-0.40],p<0.0001)was superior to the control group.According to the results of ranking:electromagnetic field therapy(82.5%)>phototherapy(82.4%)>electricalstimulation(67.9%)>ultrasound(60.8%)>cold therapy(54.6%).2)In reducing serum creatine kinase,phototherapy(SMD=-3.01,95%CI[-3.67,-2.35],p<0.0001),ultrasound(SMD=-1.34,95%CI[-2.15,-0.54],p=0.001),cold therapy(SMD=-0.17,95%CI[-0.30,-0.03],p=0.018)were better than the control group;According to the results of ranking:phototherapy(100.0%)>ultrasound(83.1%)>cold therapy(53.6%).Conclusion In relief from muscle pain,the effect of electromagnetic field therapy is the best,followed by phototherapy,electrostimulation,ultrasound,cold therapy.In the reduction of serum creatine kinase,phototherapy had the best effect,followed by ultrasound,cold therapy.
