摘 要:目的评估我国老年心衰患者营养障碍的患病情况。方法检索数据库中国知网、万方数据库、PubMed、Embase、Cochrane Library和Web of Science中关于我国老年心衰患者营养状况的相关文献,检索时间为自数据库建立后至2022年8月,经两名研究者筛选文献、提取信息并对纳入文献进行偏倚风险评价后,采用Stata16.0软件进行Meta分析。结果共纳入24篇文献,纳入研究的总样本量为6090例,其中营养障碍患者3594例。Meta分析结果显示,我国老年心衰患者营养障碍患病率为58.0%(95%CI 52.0%~64.0%)。亚组分析结果显示,女性、三线城市、疫情之后、慢性心衰患者、年龄≥65岁和≥70岁、心功能Ⅱ~Ⅳ的老年心衰患者营养障碍患病率较高。结论我国老年心衰患者营养障碍患病率较高,研究者应制定营养障碍相关概念和针对不同类型老年心衰患者营养状况工具,医护工作者可帮助老年心衰患者建立正确的营养认知和健康饮食习惯。老年心衰患者可通过非药物干预措施改善自身营养状况,提高生活质量。Objective To evaluate the prevalence of nutritional disorders in elderly patients with heart failure in China.Methodss CNKI,WanFang,PubMed,Embase,Cochrane Library and Web of Science were searched for relevant literatures on the nutritional status of elderly patients with heart failure in China from the establishment of the database to August 2022.After literature screening,information extraction and bias risk assessment of the included literatures,Statal6.O software was used for meta-analysis.Results A total of 24 literatures were included,including a total sample size of 6090 patients,including 3594 patients with nutritional disorders.The results of meta-analysis showed that the prevalence of nutritional disorders in elderly patients with heart failure was 58.0%(95%CI 52.0%~64.0%).The results of subgroup analysis showed that the prevalence of nutritional disorders was higher in female,third-tier cities,after the epidemic,chronic heart failure patients,aged≥65 years and≥70 years,and Chinese elderly heart failure patients with heart function II~IV.Conclusion The prevalence of nutritional disorders in elderly patients with heart failure is high.Researchers should develop related concepts of nutritional disorders and nutritional tools for different types of elderly patients with heart failure.Medical workers should help elderly patients with heart failure to establish correct nutritional cognition and healthy eating habits.Elderly patients with heart failure can improve their nutritional status and quality of life through nondrug interventions.
