摘  要:目的探究梅毒血清固定患者外周血Toll样受体(TLR)/核因子-κB(NF-κB)信号通路表达变化与临床特征的关系。方法选取2019年6月至2021年6月上海市徐汇区大华医院收治的104例梅毒患者作为研究对象。根据常规驱梅治疗结果,将血清固定患者52例设为观察组,血清转阴患者52例设为转阴组。另选取同期健康检查的健康者52例设为对照组。比较各组外周血NF-κB、TLR2、TLR4 mRNA水平,分析其表达水平与梅毒血清固定的相关性,并绘制受试者工作特征(ROC)曲线。结果三组外周血TLR2、TLR4、NF-κB mRNA表达水平比较,差异具有统计学意义(P<0.05)。不同初始滴度患者外周血TLR2、TLR4、NF-κB mRNA表达水平比较,差异具有统计学意义(P<0.05)。Logistic回归分析显示,外周血NF-κB、TLR2、TLR4 mRNA水平是影响梅毒血清固定的独立危险因素(OR=1.605,P=0.007;OR=1.802,P=0.000;OR=1.685,P=0.003)。ROC曲线显示,外周血NF-κB、TLR2、TLR4 mRNA联合检诊断的曲线下面积为0.938。结论梅毒血清固定患者外周血TLR/NF-κB信号通路受到明显抑制,TLR2、TLR4、NF-κB mRNA可能参与了梅毒患者机体抗病毒免疫反应。Objective To explore the relationship between the expression changes of Toll-like receptor(TLR)/nuclear factor-κB(NF-κB)signaling pathway in peripheral blood and clinical characteristics of syphilis sero-fixation patients.Methods The syphilis patients admitted to Dahua Hospital,Xuhui District from June 2019 to June 2021 were selected as the research subjects.According to the results of routine anti-syphilis treatment,52 sero-fixation patients were set as the observation group,52 sero-negative patients were selected as the negative group,and 52 healthy people who had undergone health examinations during the same period were set as the control group.The NF-κB,TLR2,TLR4 mRNA levels in peripheral blood of each group were compared,the correlation between their expression levels and syphilis sero-fixation was analyzed,and receiver operating characteristic(ROC)curves were drawn.Results The expression levels of TLR2,TLR4 and NF-κB mRNA in peripheral blood of the three groups were significantly different(P<0.05).The expression levels of TLR2,TLR4 and NF-κB mRNA in peripheral blood of patients with different initial titers were significantly different(P<0.05).Logistic regression analysis showed that NF-κB,TLR2,TLR4 mRNA levels in peripheral blood were independent risk factors for syphilis sero-fixation(OR=1.605,P=0.007;OR=1.802,P=0.000;OR=1.685,P=0.003).The ROC curve showed that the area under the curve of the combined detection of NF-κB,TLR2 and TLR4 mRNA in peripheral blood was 0.938.Conclusions The TLR/NF-κB signaling pathway in peripheral blood of syphilis sero-fixation patients is significantly inhibited,and TLR2,TLR4 and NF-κB mRNA may be involved in the antiviral immune response of syphilis patients.
