成人水痘-带状疱疹病毒感染致暴发性紫癜1例

摘　　要：水痘-带状疱疹病毒(VZV)是一种可感染人类的疱疹病毒,表现为水痘或带状疱疹。本文报告1例男性患者,32岁,表现为反复皮疹4周,伴随症状包括腹痛、高热、足底血疱、血小板减少、纤维蛋白原降低、D-二聚体升高、铁蛋白升高、抗磷脂抗体阳性。皮肤病理明确病毒性皮疹,血液化验VZV脱氧核糖核酸(DNA)阳性。患者被诊断为VZV感染合并暴发性紫癜,在接受抗病毒、抗细菌、丙种球蛋白、糖皮质激素、抗凝等治疗后,患者的病情及实验室指标均有所好转。Varicella-zoster virus(VZV)is a contagious herpes virus that causes chickenpox and herpes zoster(shingles).We report a 32-year-old male patient who presented with recurrent rash for 4 weeks,accompanied by abdominal pain,high fever,plantar blister,thrombocytopenia,decreased fibrinogen,elevated D-dimer,elevated ferritin,and positive antiphospholipid antibodies.The skin pathological confirmed diagnosis was viral rash.In addition,blood test was positive for VZV DNA.The patient was diagnosed with VZV infection complicated with purpura fulminans.The patient's condition was controlled with laboratory tests improved,following treatment with antivirals,antibacterials,intravenous immunoglobulin(IVIG),glucocorticoids,anticoagulation.etc.
