摘 要:目的:探讨维生素D(vitamin D,Vit D)对60岁以上老年女性2型糖尿病(type 2diabetic,T2DM)患者颈动脉病变的影响,为改善T2DM并发症的临床治疗提供依据。方法:回顾分析北京市垂杨柳医院2020年1月至2020年12月,收治的175例符合标准的老年女性T2DM患者的临床资料。依据颈动脉超声测量颈动脉内-中膜厚度(carotid intravagal media thickness,CIMT),分为正常组(A组):CIMT<1.0mm;颈动脉斑块组(B组):局限性CIMT≥1.5mm。二元Logistic回归分析颈动脉病变的影响因素。结果:A组69例,占39.4%;B组106例,占60.6%。A组的T2DM病程、合并高血压比例、Hb A1C、FPG、TG、HCY明显低于B组,而HDL-C、25(OH)D明显高于B组,差异有统计学意义(P<0.05)。Logistic回归分析结果显示:T2DM病程、合并高血压、TG、HCY是老年女性T2DM患者颈动脉病变的重要危险因素(P<0.05),25(OH)D是老年女性T2DM患者颈动脉病变的重要保护因素(P<0.05)。结论:25(OH)D是老年女性T2DM患者颈动脉病变的保护因素,关注老年女性T2DM患者的维生素D缺乏现状并积极纠正治疗。Objective:The objective of this study is to examine the influence of Vitamin D(VitD)on carotid artery disease in elderly women aged 60 and above with Type 2 diabetes mellitus(T2DM),and to generate clinical evidence to support the alleviation of T2DM related complications.Methods:A retrospective analysis of clinical data of 175 eligible elderly women with T2DM who were admitted to Beijing Chuiyangliu Hospital from January 2020 to December 2020 was carried out.Participants were divided into two groups based on the carotid intravagal media thickness(CIMT)measured by carotid ultrasound:Group A with a CIMT<1.0mm as the normal group,and Group B with a limited CIMT≥1.5mm as the carotid plaque group.Binary Logistic regression was employed to investigate the factors influencing carotid artery disease.Results:A total of 175 elderly women with T2DM were divided into two groups:Group A(69 cases,accounting for 39.4%)and Group B(106 cases,accounting for 60.6%).The results of the study showed that the levels of duration of T2DM,proportion of combining hypertension,HbA1c,FPG,TG and HCY in Group A were significantly lower than those in Group B,while HDL-C and 25(OH)D were significantly higher in Group A than in Group B(P<0.05).Logistic regression analysis revealed that duration of T2DM,combining hypertension,TG and HCY were important risk factors for carotid artery disease in elderly women with T2DM(P<0.05),while 25(OH)D was an important protective factor(P<0.05).Conclusions:This research determined that 25(OH)D can act as a safeguard against carotid artery disease in elderly female patients with Type 2 Diabetes Mellitus.Clinicians should be aware of vitamin D insufficiency in elderly female patients with T2DM and take steps to rectify it.
