县域医共体组织脆弱性评价指标体系的构建

摘　　要：目的 构建一套科学合理的医共体组织脆弱性评价指标体系,为医共体组织脆弱性评价提供科学管理工具。方法 采用文献分析法和专家咨询法完成医共体组织脆弱性评价指标体系的筛选和评估,采用层次分析法确定各项指标的权重。结果 最终构建的指标体系包括3项一级指标、10项二级指标和39项三级指标。两轮专家咨询的积极系数分别为80%、75%,专家协调系数为0.177(P<0.001),3项一级指标的专家权威系数分别为0.900、0.809和0.901。一级指标中诊疗服务连续性权重(0.582 6)最高,二级指标中互联互通权重(0.514 6)最高,三级指标中基层医疗机构人员到牵头医疗机构培训进修次数权重(0.095 2)最高。结论 专家权威程度、协调程度和意见集中程度均较高,医共体组织脆弱性评价指标体系具有较高可信度,可作为评估医共体组织脆弱性的工具。Objective To construct a set of scientific and reasonable evaluation index system for organizational vul⁃nerability of the healthcare alliance,and to provide scientific management tools for the organizational vulnerability evaluation of the healthcare alliance.Methods The literature analysis method and expert consultation method were used to complete the screening and evaluation of the vulnerability evaluation index system of the healthcare alliance,and the analytic hierarchy process was used to determine the weight of each index.Results A set of indicators sys⁃tem,which consisted of 3 first-level indicators,10 second-level indicators and 39 third-level indicators,was finally constructed.The positive coefficients of the two rounds of expert consultation were 80%and 75%respectively,the coordination coefficient of experts was 0.177(P<0.001),the expert authority coefficients of the first-level indica⁃tors were 0.900,0.809 and 0.901,respectively.The weight of the continuity of diagnosis and treatment services(0.5826)was relatively higher in the first-level indicator,the weight of interconnectivity(0.5146)was relatively higher in the second-level indicator.In the third-level indicator,the weight of the number of primary medical institu⁃tions personnel training in leading medical institutions(0.0952)was relatively higher.Conclusion The degree of au⁃thority,coordination and opinion concentration of experts are all high.And the evaluation index system for the organi⁃zational vulnerability of the healthcare alliance has high credibility,and can be used as a tool to evaluate the organiza⁃tional vulnerability of the compact healthcare alliance.
