数字化X线影像检查在踝关节损伤诊断中的价值分析

摘　　要：目的:探讨数字化X线影像检查在踝关节损伤诊断中的价值。方法:选取2022年1—12月北京林业大学社区卫生服务中心收治的200例踝关节外伤患者作为研究对象,均行电子计算机断层扫描(CT)、数字化X线影像技术检查。以CT检查结果为金标准,分析数字化X线影像技术检查准确性,并根据患者踝关节负重情况分为负重组与非负重组,比较两组数字化X线影像技术检查相关参数结果。结果:200例踝关节外伤患者CT检查结果显示,非踝关节损伤187例,踝关节损伤13例,其中踝关节骨折10例,踝关节韧带损伤3例。经过数字化X线影像技术检查,发现踝关节损伤12例,非踝关节损伤188例,经过检验发现,数字化X线影像检查技术的灵敏度、特异度、阳性预测值、阴性预测值、诊断符合率分别为92.86%、100.00%、100.00%、99.47%、99.50%;非负重组内踝尖水平腓骨宽度、内踝尖水平内踝尖端至腓骨、内外踝尖端的垂直距离均大于负重组,差异有统计学意义(P<0.001)。结论:踝关节损伤诊断中使用数字化X线影像检查诊断准确性较高。Objective:To explore the value of digital X-ray imaging examination in the diagnosis of ankle joint injury.Methods:From January 2022 to December 2022,200 patients with ankle joint trauma admitted to the Community Health Service Center of Beijing Forestry University were selected as the study subjects.All the patients underwent computed tomography(CT)scan and digital X-ray imaging examination.The CT examination results were used as the gold standard to analyze the accuracy of digital X-ray imaging examination.The patients were divided into weight-bearing group and non-weight-bearing group according to the weight bearing of the patient's ankle joint.The relevant parameter results of digital X-ray imaging examination in two groups were compared.Results:CT examination results of 200 patients with ankle joint trauma showed that,there were 187 cases of non-ankle joint injury and 13 cases of ankle joint injury,including 10 cases of ankle fracture and 3 cases of ankle ligament injury.After digital X-ray imaging examination,there were 12 cases of ankle joint injury and 188 cases of non-ankle joint injury.The sensitivity,specificity,positive predictive value,negative predictive value,and diagnostic compliance rate of digital X-ray imaging examination were 92.86%,100.00%,100.00%,99.47%and 99.50%,respectively.The horizontal fibula width of the medial malleolus tip,the distance from the medial malleolus tip to the fibula,and the vertical distance between medial and lateral malleolus tip in non-weight-bearing group were higher than those in the weight-bearing group,and the differences were statistically significant(P<0.001).Conclusion:The diagnostic accuracy of digital X-ray imaging examination is high in the diagnosis of ankle joint injury.
