基于加速康复外科理念的围术期护理对老年髋部骨折患者的应用效果分析

摘　　要：目的:探讨基于加速康复外科理念的围术期护理对老年髋部骨折患者的应用效果。方法:选取2020年6月—2021年5月在广东省肇庆市第一人民医院接受手术治疗的62例老年髋部骨折患者作为研究对象,采用随机数字表法分为观察组与常规组,各31例。常规组给予围术期常规护理,观察组给予基于加速康复外科理念的围术期护理。比较两组护理效果。结果:护理后1、3、5 d,观察组疼痛评分低于常规组,差异有统计学意义(P<0.05)。观察组髋关节恢复优良率高于常规组,差异有统计学意义(P=0.046)。观察组首次下床活动时间及住院时间均短于常规组,差异有统计学意义(P<0.05)。观察组术后并发症发生率低于常规组,差异有统计学意义(P=0.031)。结论:基于加速康复外科理念的围术期护理能够缩短老年髋部骨折患者首次下床活动时间及住院时间,加快髋关节功能恢复,减轻疼痛程度,降低术后并发症发生率。Objective:To explore the application effect of perioperative nursing based on the concept of enhanced recovery after surgery in the elderly patients with hip fracture.Methods:Sixty-two elderly patients with hip fracture who underwent surgical treatment in Zhaoqing First People's Hospital of Guangdong Province from June 2020 to May 2021 were selected as the study subjects.They were divided into observation group and routine group by random number table method,with 31 cases in each group.The routine group was given perioperative routine nursing,and the observation group was given perioperative nursing based on the concept of enhanced recovery after surgery.The nursing effects were compared between the two groups.Results:At 1,3 and 5 d after nursing,the pain scores in the observation group were lower than those in the routine group,and the difference was statistically significant(P<0.05).The excellent and good rate of hip joint recovery in the observation group was higher than that in the routine group,and the difference was statistically significant(P=0.046).The first ambulation time and the length of hospital stay in the observation group were shorter than those in the routine group,and the differences were statistically significant(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the routine group,and the differences were statistically significant(P=0.031).Conclusion:Perioperative nursing based on the concept of enhanced recovery after surgery can shorten the reduce the first ambulation time and the length of hospital stay of elderly patients with hip fracture,accelerate the recovery of hip joint function,reduce the degree of pain,and reduce the incidence of postoperative complications.
