活血止痛汤联合脱水剂治疗胫骨平台骨折术后患肢肿胀的临床疗效分析

摘　　要：目的研究活血止痛汤联合脱水剂治疗胫骨平台骨折术后患肢肿胀的临床疗效。方法选取本院收治的60例胫骨平台骨折术后患肢肿胀患者(2020年8月至2022年7月),对照组(30例,常规脱水治疗)和观察组(30例,活血止痛汤联合脱水剂治疗)。结果活血止痛汤联合脱水剂治疗下胫骨平台骨折术后患肢肿胀患者中医证候疗效高于常规脱水治疗,对比结果差异显著(χ^(2)=4.952,P<0.05)。活血止痛汤联合脱水剂治疗下胫骨平台骨折术后患肢肿胀患者疼痛缓解时间、肿胀缓解时间住院时间低于常规脱水治疗,对比结果差异显著(t=28.491、19.245、24.016,P<0.05)。活血止痛汤联合脱水剂治疗下胫骨平台骨折术后患肢肿胀患者VAS疼痛评分为(0.52±0.10)分,低于常规脱水治疗,对比结果差异显著(t=7.396,P<0.05)。活血止痛汤联合脱水剂治疗下胫骨平台骨折术后患肢肿胀患者患肢肿胀严重程度评分为(0.68±0.15)分,低于常规脱水治疗,对比结果差异显著(t=6.823,P<0.05)。活血止痛汤联合脱水剂治疗下胫骨平台骨折术后患肢肿胀患者白细胞介素6、C反应蛋白指标分别为(0.42±0.26)ng/mL、(3.82±1.14)mg/L,低于常规脱水治疗,对比结果差异显著(t=9.418、15.680,P<0.05)。活血止痛汤联合脱水剂治疗下胫骨平台骨折术后患肢肿胀患者血浆黏度、高切血浆黏度分别为(1.18±0.35)mPa•s、(5.29±0.48)mPa•s,低于常规脱水治疗,对比结果差异显著(t=11.039、10.407,P<0.05)。活血止痛汤联合脱水剂治疗下胫骨平台骨折术后患肢肿胀、疼痛、畸形中医证候积分分别为(0.38±0.17分)、(0.26±0.10)分、(0.30±0.08)分,低于常规脱水治疗,对比结果差异显著(t=7.927、6.630、6.524,P<0.05)。活血止痛汤联合脱水剂治疗下胫骨平台骨折术后患肢肿胀患者胃肠道反应、头痛、头晕不良反应发生率为3.33%,低于常规脱水治疗,对比结果差异显著(χ^(2)=4.821,P<0.05)。结论活血�Objective To study the clinical efficacy of Huoxue zhitong decoction combined with dehydrating agent in treating swelling of the affected limb after tibial plateau fracture.Methods Sixty patients with swollen affected limbs after tibial plateau fractures admitted to our hospital(August 2020 to July 2022)were selected from the control group(30 patients,treated with conventional dehydration)and the observation group(30 patients,treated with Huoxue zhitong decoction combined with dehydrating agents).Results The efficacy of combining Huoxue zhitong decoction with dehydration agent in treating patients with swelling of the affected limb after lower tibial plateau fracture was higher than that of conventional dehydration treatment with a significant difference in the comparison results(χ^(2)=4.952,P<0.05).The pain relief time and swelling relief time in hospital for patients with swollen affected limbs after lower tibial plateau fracture treated with Huoxue zhitong decoction combined with dehydration agent were lower than those treated with conventional dehydration,and the difference in the comparison results was significant(t=28.491,19.245,24.016,P<0.05).The VAS pain score of patients with swelling of the affected limb after lower tibial plateau fracture treated with Huoxue zhitong decoction combined with dehydration was(0.52±0.10),which was lower than that of conventional dehydration treatment,and the difference in the comparison results was significant(t=7.396,P<0.05).The severity score of swelling of the affected limb in patients with lower tibial plateau fracture treated with Huoxue zhitong decoction combined with dehydration was(0.68±0.15),which was lower than that of conventional dehydration treatment,and the difference in the comparison results was significant(t=6.823,P<0.05).The indexes of interleukin 6 and C-reactive protein were(0.42±0.26)ng/mL and(3.82±1.14)mg/L,respectively,in patients with swelling of the affected limb after lower tibial plateau fracture treated with Huoxue zhitong decoction combined
