摘　　要：目的为推动抗菌药物在临床使用的规范化、合理化。方法2021年1月至2022年8月,每月从各科室随机抽取出院病历15份(不足15份则全部抽取)进行点评,每月抽取的科室为6~7个,本次抽取的病历共1810份,对不合理用药类型进行分析。结果在1810份出院病历中,共发现77份报告出现抗菌药物的不合理应用情形,合理率为95.75%,不合理类型主要有应用抗菌药物证据不足、使用的抗菌药物种类不适宜、围手术期抗菌用药维持时间不适宜等。结论药剂科可协同质控行政部门,一起干预抗菌药物的不合理应用状况,临床药师利用自身的专长,为临床医师提供切实可行的药物治疗建议。Objective To promote the standardization and rationalization of the clinical application of antibacterial drugs.Methods From January 2021 to August 2022,15 medical records(or all of them if less than 15)were randomly selected from each department every month for comment.Six to seven departments were selected every month.A total of 1810 medical records were selected this time.The types of irrational drug use were analyzed.Results In 1810 discharged medical records,77 patients were found to have irrational use of antibacterial drugs,with a reasonable rate of 95.75%.The main types of irrational use were insufficient evidence of using antibacterial drugs,inappropriate selection of antibacterial drugs,inappropriate timing of administration,inappropriate maintenance time of perioperative antibacterial drugs,inappropriate frequency of administration,inappropriate dosage,and insufficient indications for combined use of antibacterial drugs.Conclusion Pharmaceutical departments can cooperate with quality control departments to intervene in the irrational use of antibacterial drugs.Clinical pharmacists should make use of their own expertise to provide practical pharmaceutical treatment plans for clinical use and ensure the safety of drug use of patients.
