摘  要:目的探讨抗炎合剂对脓毒血症大鼠胃肠黏膜屏障功能的影响。方法72只SD大鼠,随机选取18只作为对照组,其余54只采用盲肠结扎穿孔法建立脓毒血症模型并分成模型组、抗炎合剂组和阳性对照组,所有大鼠均于建模前72 h开始每天1次灌胃相应药物进行干预、直至建模成功(术后6 h);抗炎合剂组给予抗炎合剂9 mL/(kg·d),阳性对照组给予头孢哌酮舒巴坦0.2 g/(kg·d),对照组、模型组予等量生理盐水灌胃;于建模后24 h时取血检测各组大鼠血清白细胞介素1β(IL-1β)、肿瘤坏死因子α(TNF-α)、干扰素γ(INF-γ)、D-乳酸、胆囊收缩素(CCK)、内毒素(ET)水平,取大鼠降结肠粪便中大肠杆菌及乳酸杆菌菌落计数,取小肠组织HE染色观察组织学变化、检测紧密连接蛋白-1(ZO-1)及肠上皮咬合蛋白(Occludin)表达水平。结果抗炎合剂组大鼠血清INF-γ、IL-1β、TNF-α、D-乳酸、CCK、ET水平及大鼠降结肠粪便中大肠杆菌菌落数均低于模型组,乳酸杆菌菌落数高于模型组,差异有统计学意义(P<0.05);HE染色结果显示,模型组大鼠小肠组织细胞水肿明显,可见大量炎症细胞浸润,抗炎合剂组大鼠及阳性对照组小肠细胞水肿减轻,少量炎症细胞浸润,阳性对照组改善程度更明显;抗炎合剂组大鼠小肠组织ZO-1、Occludin蛋白表达水平高于模型组,差异有统计学意义(P<0.05)。结论抗炎合剂能够改善脓毒血症大鼠炎症反应及胃肠黏膜屏障功能,维持肠道菌群平衡,对大鼠小肠组织具有一定的保护作用。Objective To investigate the effect of anti-inflammatory mixture on gastrointestinal mucosal barrier function in sepsis rats.Methods A total of 72 SD rats were selected,18 were randomly selected as control group,and the rest were treated with cecal ligation and perforation to establish sepsis model.One died during the modeling process,and the rest were randomly divided into 3 groups,18 in each of the model group and the anti-inflammatory agent group,and 17 in the positive group.All rats were administrated with corresponding drugs 72 h before modeling until modeling was successful(6 h after surgery).The anti-inflammatory mixture group was given 9 mL/(kg·d),the positive group was given cefoperazone sulbactam 0.2 g/(kg·d),and the control group and model group were given the same amount of normal saline ingavage.The serum levels of interleukin 1β(IL-1β),tumor necrosis factorα(TNF-α),interferonγ(INF-γ),D-lactic acid,cholecystokinin(CCK),endotoxin(ET),and the count of Escherichia coli and lactobacillus in feces of rats in descending colon were compared for 24 h.The expression levels of tight junction protein-1(ZO-1)and intestinal epithelial Occludin protein in small intestine.Results The levels of INF-γ,IL-1β,TNF-α,D-lactic acid,CCK,and ET in serum and the number of Escherichia coli colonies in feces of descending colon of rats in anti-inflammatory mixture group were lower than those in model group,while the number of lactobacillus colonies was higher than that in model group,the differences were statistically significant(P<0.05).The expression level of ZO-1 and Occludin in small intestine of anti-inflammatory mixture group was higher than that of model group,and the difference was statistically significant(P<0.05).Conclusion Anti-inflammatory mixture can improve the inflammatory response and the function of gastrointestinal mucosa in sepsis rats,maintain the balance of intestinal flora,and have a certain protective effect on the small intestine tissue of rats.
