摘　　要：目的 探究鼻渊通窍颗粒联合头孢地尼对鼻窦炎合并鼻息肉患者鼻阻力及高迁移率族蛋白B(HMGB1)、γ干扰素(IFN-γ)和白细胞介素-5(IL-5)水平的影响。方法 选取收治的80例鼻窦炎合并鼻息肉患者,分为观察组(40例)和对照组(40例),两组均行鼻内镜手术,对照组在术后采用头孢地尼治疗,观察组在对照组基础上联合鼻渊通窍颗粒。记录两组临床疗效、鼻阻力,血清HMGB1、嗜酸性粒细胞阳离子蛋白(ECP)、IFN-γ、IL-5、主观症状(VAS)评分、中医证候积分以及不良反应发生情况。结果 治疗后,观察组临床疗效及鼻阻力改善效果优于对照组,IFN-γ显著升高,且观察组高于对照组,HMGB1、ECP、IL-5、VAS评分、中医证候积分均明显下降,且观察组低于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论 采用鼻渊通窍颗粒联合头孢地尼治疗鼻窦炎合并鼻息肉患者疗效显著,可有效缓解临床症状,改善鼻阻力,减轻变态反应及炎症反应,安全可靠。bjective To investigate the effects of Biyuan Tongqiao granule combined with cefdinir on nasal resistance and high mobility group protein B1(HMGB1),interferon-γ(IFN-γ)and interleukin-5(IL-5)levels in patients with sinusitis and nasal polyps.Methodss A total of 80 patients with sinusitis complicated with nasal polyps were selected and divided into the study group(40 cases)and the control group(40 cases).Both groups were treated with nasal endoscopic surgery.The control group was treated with cefdinir after surgery,while the study group was combined with Biyuan Tongqiao granule on the basis of the control group.The clinical efficacy,nasal resistance,HMGB1,eosinophil cationic protein(ECP),IFN-γ,IL-5,subjective symptom(VAS)score,TCM syndrome score and adverse reactions were recorded.Results After treatment,the clinical efficacy and nasal resistance improvement effect of the study group were better than those of the control group.IFN-γwas significantly increased,and the study group was significantly higher than the control group;and HMGB1,ECP,IL-5,VAS score and TCM syndrome score were significantly reduced,and the study group were signficantly lower than the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Biyuan Tongqiao granule combined with cefdinir is effective in the treatment of rhinosinusitis complicated with nasal polyps,which can effectively relieve clinical symptoms,improve nasal resistance,reduce allergy and inflammation,and is safe and reliable.
