单孔腹腔镜下输卵管吻合术后妊娠结局的影响因素

摘　　要：目的 分析单孔腹腔镜下输卵管吻合术后妊娠结局的影响因素。方法 根据术后妊娠结局不同,行单孔腹腔镜下输卵管吻合术的输卵管绝育患者217例分为妊娠组(113例)和未妊娠组(104例),比较两组妊娠结局相关指标和围手术期情况,并分析术后妊娠结局的影响因素。结果两组民族和婚姻状况比较有统计学差异(P<0.05)。妊娠组腹部手术史比例低于未妊娠组(P<0.05)。妊娠组术中吻合率高于未妊娠组(P<0.05)。焦虑患者妊娠率低于无焦虑患者(P<0.05)。民族、腹部手术史、术中吻合情况及术后焦虑是影响输卵管绝育患者术后妊娠的独立因素(P<0.05)。结论 单孔腹腔镜下行输卵管吻合术有助于提高输卵管绝育患者术后妊娠率,且妊娠结局受多因素影响。Objective To analyze the influencing factors of pregnancy outcomes after single hole laparoscopic tubal anastomosis.Methods According to the different pregnancy outcomes after operation,217patients with tubal sterilization underwent single hole laparoscopic tubal anastomosis were divided into groups of A(pregnant,113cases)and B(non-pregnant,104cases).The relevant indexes of pregnancy outcomes and perioperative conditions of two groups were compared and the influencing factors were analyzed.Results There were significant differences in ethnic and marital status between the two groups(P<0.05).The proportion of the patients with abdominal operation history in group A was lower than that in group B(P<0.05).The anastomosis rate of group A was higher than that of group B(P<0.05).The pregnancy rate of anxious patients was lower than that of those without anxiety(P<0.05).Ethnic,abdominal operation history,intraoperative anastomosis and postoperative anxiety were the independent factors affecting postoperative pregnancy in the patients with tubal sterilization(P<0.05).Conclusion Single hole laparoscopic tubal anastomosis is helpful to improve the pregnancy rate of the patients after tubal sterilization,in whom the pregnancy outcomes are affected by many factors.
