摘 要:目的比较三踝骨折治疗中后踝以支撑接骨板和中空拉力螺钉固定的临床效果和影像学表现,为踝关节骨折治疗提供参考意见。方法回顾性分析2015年5月至2018年1月玉溪市中医医院收治的符合纳入标准的40例三踝骨折患者的临床资料。按后踝固定方法分为支撑接骨板固定组(17例)和中空拉力螺钉固定组(23例),并对相关的术后及随防的临床和影像学结果进行分析。结果40例患者均获得随访,所有患者骨折均愈合良好,所有患者对踝关节功能恢复均较满意。结论后外侧入路支撑接骨板和中空拉力螺钉固定是三踝骨折中后踝骨折常用的固定方法,两种方法均可获得较好的临床和影像学结果。与中空拉力螺钉相比,支撑接骨板固定可处理更复杂的后踝骨折,可获得更稳定的固定,进而可达到术后早期功能锻炼的目的。Objective To compare the clinical effects and imaging findings of posterior malleolus fixation with brace plate and hollow lag screw in the treatment of trimalleolar fracture,and to provide reference for the treatment of ankle fracture.Methods A retrospective analysis was performed on 40 patients with trimalleolar fracture who met the inclusion criteria and were admitted to the orthopaedics Department of Yuxi Hospital of Traditional Chinese Medicine from May 2015 to January 2018.According to the method of posterior malleolus fixation,the patients were divided into the brace plate fixation group(17 cases)and the hollow lag screw fixation group(23 cases),and the related postoperative and follow-up clinical and imaging results were analyzed.Results All the 40 patients were followed up.All the fractures healed well and all the patients were satisfied with the functional recovery of ankle joint.Conclusion Posterolateral approach brace plate and hollow lag screw fixation are common fixation methods for posterior malleolar fracture in trimalleolar fracture.Both methods can obtain good clinical and imaging results.Compared with hollow lag screws,brace plate fixation can be used to treat more complex posterior malleolus fractures and achieve more stable fixation,thus achieving early postoperative functional exercise.
