摘　　要：目的探讨对急性跟腱断裂患者采用卵圆钳辅助小切口改良Kessler缝合治疗的临床效果。方法选择射阳县人民医院2019年1月至2022年4月收治的急性跟腱断裂患者70例进行研究,以随机数表法分为对照组(传统切开Kessler缝合法治疗,n=35)与观察组(采用卵圆钳辅助小切口改良Kessler缝合治疗,n=35)。治疗后,比较两组手术相关指标,以Arner-Lindholm标准评估临床疗效,采用跟腱断裂评分(achilles tendon total rupture score,ATRS)、美国足踝外科协会(American Orthopaedic Foot and Ankle Society,AOFAS)踝-后足评分评估跟腱与关节功能,另比较两组并发症发生率。结果观察组手术时间、切口长度、住院时间均短于对照组(P<0.05)。观察组治疗优良率高于对照组(P<0.05)。术后3个月、6个月,观察组的ATRS与AOFAS评分均高于对照组(P<0.05)。观察组并发症总发生率低于对照组(P<0.05)。结论对急性跟腱断裂患者予卵圆钳辅助小切口改良Kessler缝合治疗,对患者损伤小,操作可靠,疗效确切,有利于患者的跟腱与踝关节功能恢复,可减少术后并发症发生风险。Objective To investigate the clinical effect of modified Kessler suture assisted by ovale forceps with small incision in patients with acute achilles tendon rupture.Methods Seventy patients with acute achilles tendon rupture admitted to the Sheyang People's Hospital from January 2019 to April 2022 were randomly divided into the control group treated with traditional incision Kessle suture(n=35)and the observation group treated with ovale forceps assisted small incision modified Kessler suture(n=35).After treatment,compare the two groups of surgical related indicators.The clinical effects were evaluated by Arner-Lindholm criteria and the related indexes were compared between the two groups,achilles tendon and joint function were evaluated using the achilles tendon rupture score(ATRS)and the American Society for Foot and Ankle Surgery(AOFAS)ankle hindfoot score,and complication rates were compared between the two groups.Results The operation time,incision length and hospital stay in the observation group were shorter than those in the control group(P<0.05).Excellence rate was higher in the observation group than in the control group(P<0.05).ATRS and AOFAS score in the observation group were higher than those in the control group 3 months and 6 months after surgery(P<0.05).The incidence of complications in observation group was lower than that in control group(P<0.05).Conclusion For patients with acute achilles tendon rupture,modified Kessler suture assisted by ovale forceps with small incision has small damage,reliable operation and accurate efficacy,which is conducive to the functional recovery of achilles tendon and ankle,and can reduce the risk of postoperative complications,and is worthy of promotion.
