经皮内固定支架治疗骨盆前环不稳定型骨折的临床研究

摘　　要：目的探讨经皮内固定支架(INFIX)治疗骨盆前环不稳定型骨折的效果和安全性。方法回顾性分析新乡市第一人民医院骨科2018-01-2021-12行INFIX治疗的18岁以上骨盆前环不稳定型骨折患者的临床资料。记录患者的一般资料、骨折类型、损伤严重度评分、并发症和手术时间。取出经皮INFIX装置后对患者进行随访12个月,评价爱荷华骨盆评分(IPS)。结果本组48例患者的IPS评分为82.7分(范围:55~100分)。其中,优18例(85~100分),良21例(70~84分),可7例(55~69分),差2例(55分)。优良率达81.3%。置入经皮INFIX后最常见的并发症为股外侧皮神经损伤11例(22.9%),2例(4.2%)发生感染,5例(10.4%)发生异位骨化。结论经皮INFIX是治疗骨盆前环不稳定型骨折的一种安全、有效的方法。Objective To investigate the effect and safety of percutaneous internal fixation(INFIX)in the treatment of unstable fractures of the anterior pelvic ring.Methods The clinical data of patients over 18 years old with unstable anterior pelvic ring injuries who underwent percutaneous INFIX treatment in the Department of Orthopedics of Xinxiang the First People's Hospital from January 2018 to December 2021 were retrospectively analyzed.The patient's general information,fracture type,injury severity score,complications and operation time were recorded.All patients were followed up for 12 months after removal of the percutaneous INFIX device to evaluate the Iowa pelvic score(IPS).Results The IPS score of the 48 patients in this group was 82.7 points(range:55~100 points),of which 18 cases were excellent(85~100 points),21 cases were good(70~84 points),and 7 cases were fair.(55~69 points),poor in 2 cases(55 points).The good rate is 81.3%.The most common complications after percutaneous INFIX implantation were lateral femoral cutaneous nerve injury in 11 cases(22.9%);infection in 2 cases(4.2%);and heterotopic ossification in 5 cases(10.4%).Conclusion Percutaneous INFIX is a safe and effective method for treating unstable injuries of the anterior pelvic ring.
