摘　　要：目的探讨MR磁敏感加权成像(SWI)的动脉磁敏感征(SVS)在急性大脑中动脉闭塞症(AMCAO)血管内治疗术前对血栓检测的准确性及其测量血栓位置和长度的价值。方法回顾性分析52例AMCAO患者的SWI及数字减影血管造影(DSA)的影像学资料。以DSA测量血栓位置及血栓长度为金标准,计算SWI的血栓检出率;用Pearson相关分析评价SVS与DSA测量血栓位置之间的相关性;分析比较SVS与DSA测量血栓长度的相关性。结果SVS、DSA对血栓显示率分别为98.1%、100%。SVS与DSA测量血栓位置(Pearson相关系数0.983,P<0.01)有很强的相关性。SVS测量血栓长度与DSA(Pearson相关系数0.919,P<0.001)有很强的相关性。结论SWI的SVS发现AMCAO血栓敏感性高、测量血栓位置和长度准确性高,对血管内治疗术前有较高的指导价值。Objective To investigate the accuracy of susceptibility vessel sign(SVS)of MR susceptibility weighted imaging(SWI)for thrombus detection before endovascular treatment of acute middle cerebral artery occlusion(AMCAO)and its value in measuring thrombus location and length.Methods The imaging data of SWI and digital subtraction angiography(DSA)of 52 patients diagnosed with AMCAO were analyzed retrospectively.Taking DSA measurements of thrombus location and thrombus length as the gold standard,SwI thrombus detection rate was calculated;Pearson correlation analysis was used to evaluate the correlation between SVS and DSA measurements of thrombus location;The correlation between SVS and DSA measurements of thrombus length was analyzed and compared.Results The thrombosis rate of SVS and DSA were 98.1%,100%,respectively.SVS correlated strongly with DSA measurements of thrombus location(Pearson correlation coefficient 0.983,P<0.01).SVS measurements of thrombus length correlated strongly with DSA(Pearson correlation coefficient 0.919,P<0.001).Conclusion The SWI-SVS finds high sensitivity of AMCAO thrombus,high accuracy of measuring thrombus location and length,and high value for preoperative guidance of endovascular treatment.
