摘  要:目的探讨不同低剂量扫描条件联合高级模拟迭代重建(ADMIRE)在咯血患者胸部CT血管成像(CTA)的对比研究。方法前瞻性选取采用第3代双源CT行胸部CTA检查的172例咯血患者,随机采用数字表法分为A、B、C、D4组,每组各43例。A组行常规剂量扫描,B、C、D组均行低剂量扫描。A组扫描条件采用管电压120kV,管电流150mAs,滤波反投影(FBP)重建图像;B组扫描条件采用管电压120kV,管电流90mAs,ADMIRE3级重建图像;C组扫描条件采用管电压100kV,管电流150mAs,ADMIRE3级重建图像;D组扫描条件采用管电压100kV,管电流90mAs,ADMIRE3级重建图像。比较4组图像质量主观评分、客观评价及辐射剂量的差异。结果4组血管CT值、背景噪声(BN)、信噪比(SNR)、对比噪声比(CNR)比较差异有统计学意义(P<0.05)。4组辐射剂量比较差异有统计学意义(P<0.05),其中D组辐射剂量最低。4组图像主观评分比较差异无统计学意义(P>0.05),均满足临床诊断要求,其中C组评分最高。结论不同低剂量扫描条件联合ADMIRE获得的图像质量均能满足临床诊断要求,并且不同程度地降低咯血患者胸部CTA的辐射剂量。采用管电压100kV、管电流150mAs扫描条件联合ADMIRE获得的图像质量最优,但辐射剂量降低幅度不大。采用管电压100kV、管电流90mAs扫描条件联合ADMIRE获得的图像质量能够满足临床诊断要求,且产生的辐射剂量最低。Objective To compare chest computed tomography angiography(CTA)in patients with hemoptysis under different low-dose scanning conditions combined with advanced modeled iterative reconstruction(ADMIRE).Methods A total of 172 patients with hemoptysis who underwent chest CTA examination by third-generation dual-source CT were prospectively selected and randomly divided into four groups A,B,C and D by number table method,with 43 cases in each group.Group A(tube voltage of 120 kV,tube current of 150 mAs)received conventional-dose scanning to reconstruct the image with filtered back projection(FBP),while group B(tube voltage of 120 kV,tube current of 90 mAs),C(tube voltage of 100 kV,tube current of 150 mAs)and D(tube voltage of 100 kV,tube current of 90 mAs)received low-dose scanning to reconstruct the image with ADMIRE 3.The differences in the subjective score and objective evaluation of image quality and the radiation dose were compared among the four groups.Results There were statistically significant differences in CT value,background noise(BN),signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)among the four groups(P<0.05).There was a statistically significant difference in radiation dose among the four groups(P<0.05)with the lowest radiation dose in group D.There was no statistically significant difference in the subjective score among the four groups(P>0.05),all met the requirements of clinical diagnosis,and the group C had the highest score.Conclusion The image quality of different low-dose scanning conditions combined with ADMIRE can meet the requirements of clinical diagnosis and reduce the radiation dose of chest CTA in patients with hemoptysis with varying degrees.The best image quality is obtained by adopting the scanning condition of tube voltage of 100 kV and tube current of 150 mAs combined with ADMIRE,but the reduction of radiation dose is not large.The image quality obtained by adopting the scanning condition of tube voltage of 10o kV and tube current of 90 mAs combined with ADMIRE can meet the
