扶正通络中药雾化吸入治疗慢性阻塞性肺疾病急性加重临床研究  

Clinical efficacy of aerosol inhalation of Fuzheng Tongluo Chinese medicine on acute exacerbation of chronic obstructive pulmonary disease

摘　　要：目的观察扶正通络中药雾化吸入治疗慢性阻塞性肺疾病急性加重(AECOPD)患者的临床疗效。方法将207例AECOPD患者采用简单随机抽样法随机分为2组,2组均予常规基础治疗,对照组115例予吸入用布地奈德混悬液雾化吸入治疗,治疗组92例予扶正通络中药雾化吸入,2组均连续治疗30天后统计疗效。比较2组治疗前后COPD患者自我评估测试(CAT)、改良呼吸困难(mMRC)评分,血清白细胞介素6(IL-6)、超氧化物歧化酶(SOD)、C反应蛋白(CRP)水平,痰液黏蛋白5AC(MUC5AC)、中性粒细胞弹性蛋白酶(NE)水平。结果治疗组显效率及总有效率分别为75.00%(69/92),95.65%(88/92),对照组分别为51.30%(59/115)、82.61%(95/115),2组显效率及总有效率比较差异均有统计学意义(P<0.05),治疗组均高于对照组。治疗后2组CAT、mMRC评分均较本组治疗前降低(P<0.05),且治疗组均低于对照组(P<0.05)。治疗后2组FVC、FEV 1、FEV 1/FVC均较本组治疗前升高(P<0.05),且治疗组均高于对照组(P<0.05)。治疗后2组IL-6、CRP水平均较本组治疗前降低(P<0.05),SOD水平升高(P<0.05),且治疗组IL-6、CRP水平低于对照组(P<0.05),SOD水平高于对照组(P<0.05)。治疗后2组MUC5AC及NE水平均较本组治疗前降低(P<0.05),且治疗组均低于对照组(P<0.05)。结论扶正通络中药雾化吸入治疗AECOPD临床疗效显著,可改善患者肺功能,控制炎性反应,改善气道黏液高分泌状态。Objective To observe the Clinical efficacy of aerosol inhalation of Fuzheng Tongluo Chinese medicine on acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 207 AECOPD patients were randomly assigned into the treatment group(n=92)and the control group(n=115).All patients were managed by conventional basic treatment,and those in the treatment group and the control group were additionally treated with aerosol inhalation of Fuzheng Tongluo Chinese medicine and budesonide suspension liquid,respectively.Treated for 30 days,the aim was to compare COPD Assessment Test(CAT),Modified Medical Research Council(mMRC)dyspnea scale,serum interleukin-6(IL-6),superoxide dismutase(SOD),C-reactive protein(CRP),mucin 5AC(MUC5AC),neutrophil elastase(NE).The curative effect was assessed.Results The obvious effective rate(75.00%[69/92]vs 51.30%[59/115]),the overall effective rate(95.65%[88/92]vs 82.61%[95/115])in the treatment group were significantly higher than those in the control group(all P<0.05).After treatment,CAT and mMRC in the both groups were decreased(P<0.05),the treatment group was more common(P<0.05).The forced vital capacity(FVC),forced expiratory volume in one second(FEV 1),FEV 1/FVC in groups were increased,which increased in the treatment group compared with the control group(P<0.05).Reduced IL-6 and CRP,and elevated SOD were found in groups than those before treatment(P<0.05),and the different was significant between groups for the above indexes(P<0.05).After treatment,MUC5AC and NE in groups were decreased(P<0.05),which were lower in the treatment group than in the control group(P<0.05).Conclusion For AECOPD patients,aerosol inhalation of Fuzheng Tongluo Chinese medicine has significant clinical efficacy,which can improve lung function,control inflammatory response,improve airway mucus hypersecretion state.
