肝硬化食管静脉曲张程度与肝脾硬度及血流动力学的相关性

摘　　要：目的探讨肝硬化食管静脉曲张程度与肝脾硬度及血流动力学的相关性。方法选取2019年1月至2021年12月黑龙江省医院感染内科收治的60例肝硬化食管静脉曲张患者作为研究对象,根据患者食管静脉曲张程度分为轻度组(11例)、中度组(9例)和重度组(40例)。使用彩色多普勒超声对患者的肝脏和脾脏硬度和血流动力学指标等进行测量,比较三组患者的肝硬度、脾硬度、血流动力学指标。分析肝硬化食管静脉曲张程度与肝脾硬度及血流动力学的相关性。结果中度组、重度组患者的肝硬度和脾硬度强于轻度组,重度组患者的肝硬度和脾硬度强于中度组,差异有统计学意义(P<0.05);三组患者门静脉流速、肝动脉流速、脾静脉流速比较,差异无统计学意义(P>0.05);肝硬化食管胃底静脉曲张分级与肝硬度值呈正相关(P<0.05),肝硬化食管静脉曲张分级与脾硬度值呈正相关(P<0.05)。结论肝硬化食管静脉曲张程度与肝脾硬度密切相关,随着肝硬化静脉曲张程度的加重,肝脾硬度明显上升,对肝脾硬度进行检测对于肝硬化食管静脉曲张严重程度的判断具有重要意义。Objective To investigate the correlation between the degree of esophageal varices and liver spleen hardness and hemodynamics in patients with liver cirrhosis.Methods A total of 60 patients with esophageal varices due to liver cirrhosis admitted to the Department of Infection,Heilongjiang Provincial Hospital from January 2019 to December 2021 were selected as the study objects.The patients were divided into mild group(11 cases),moderate group(9 cases)and severe group(40 cases)according to the degree of esophageal varices.Color Doppler ultrasound was used to measure the liver and spleen hardness and hemodynamic indexes of the patients,and the liver hardness,spleen hardness and hemodynamic indexes of patients were compared among three groups.The correlation between the degree of esophageal varices and the hardness of liver and spleen and hemodynamics were analyzed.Results The liver hardness and spleen hardness of the moderate group and severe group were stronger than those of the mild group,and the liver hardness and spleen hardness of the severe group were stronger than those of the moderate group,the differences were statistically significant(P<0.05).There were no significant differences in portal vein velocity,hepatic artery velocity and splenic vein velocity among the three groups(P>0.05).The grading of esophageal and gastric varices in cirrhosis was positively correlated with liver hardness(P<0.05),and the grading of esophageal varices in cirrhosis was positively correlated with spleen hardness(P<0.05).Conclusion The degree of cirrhosis esophageal varices is closely related to the hardness of liver and spleen.With the aggravation of cirrhosis esophageal varices,the hardness of liver and spleen increases significantly.The detection of liver and spleen hardness is of great significance in judging the severity of cirrhosis esophageal varices.
