开窍醒神针法治疗卒中后运动性失语的临床效果

摘　　要：目的探讨开窍醒神针法治疗卒中后运动性失语(PSMA)的临床效果。方法选取2021年1月至2022年6月长春中医药大学附属医院收治的80例PSMA患者,根据随机数字表法将其分为对照组和观察组,每组40例。对照组接受常规语言康复训练和经颅直流电刺激治疗,观察组在对照组的基础上接受开窍醒神针法,两组均连续治疗6周。比较两组临床疗效和治疗前后中国康复研究中心汉语标准失语症检查(CRRCAE)、美国国立卫生研究院卒中量表(NIHSS)、卒中后失语症生活质量量表-39(SAQOL-39)评分,同时观察安全性。结果观察组临床疗效优于对照组,差异有统计学意义(P<0.05)。治疗后,两组CRRCAE中的自发言语、听语理解、复述、命名、失语商评分及SAQOL-39评分高于治疗前,NIHSS评分低于治疗前,且观察组CRRCAE各项评分及SAQOL-39评分高于对照组,NIHSS评分低于对照组,差异有统计学意义(P<0.05)。两组治疗过程中均未出现明显的不良反应。结论开窍醒神针法治疗PSMA的效果显著,能有效改善患者的失语症状和生活质量。Objective To explore the clinical effect of Kaiqiao Xinshen Acupuncture Method in the treatment of post-stroke motor aphasia(PSMA).Methods A total of 80 patients with PSMA admitted to the Affiliated Hospital of Changchun University of Chinese Medicine from January 2021 to June 2022 were selected and divided into control group and observation group according to random number table method,with 40 cases in each group.The control group received routine speech rehabilitation training and transcranial direct current stimulation treatment,and the observation group received Kaiqiao Xinshen Acupuncture Method on the basis of the control group.Both groups were treated for six weeks.The clinical efficacy were compared,and the scores of China Rehabilitation Research Center Chinese standard aphasia examination(CRRCAE),National Institutes of Health stroke scale(NIHSS),and the stroke and aphasia quality of life scale-39(SAQOL-39)before and after treatment were compared between the two groups,and the safety was also observed.Results The clinical efficacy of the observation group was better than that of the control group,and the difference was statistically significant(P<0.05).After treatment,spontaneous speech,listening comprehension,retelling,naming,aphasia quotient scores of CRRCAE,and SAQOL-39 scores in both groups were higher than those before treatment,and the NIHSS scores of the both groups were lower than those before the treatment,and the CRRCAE scores and SAQOL-39 scores of the observation group were higher than those in the control group,and the NIHSS scores were lower than those in the control group,the differences were statistically significant(P<0.05).There were no obvious adverse reactions during the treatment in both groups.Conclusion The treatment of PSMA with Kaiqiao Xinshen Acupuncture Method has a remarkable effect,which can effectively improve the aphasia symptoms and quality of life of patients.
